Özet

Amaç: Karotid arter cerrahisi uygulanan hastalarda morbidité ve mortalité oranlarının araştırılması amaçlandı. Gereç ve Yöntem: Erciyes Üniversitesi Tıp Fakültesi Göğüs ve Kalp- Damar Cerrahisi Anabilim dalında 1978 - 1997 yılları arasında karotid arter cerrahisi uygulanan 21 hasta retrospektif olarak incelendi. Onyedisi (% 81 ) erkek, dördü (% 19) ise kadın olan olguların yaş ortalaması 39.7 (en genç 8 , en yağlı 63) yıl olarak bulundu. Bulgular: Operasyon nedeni 11 ( % 52.3 ) olguda penetran yaralanma, dokuz (% 38.1) olguda arteriosklerotik lezyon, bir ( % 4.8 ) olguda Behçet hastalığına sekonder gelişen anevrizma idi. Cerrahi uygulama olarak endarterektomi sekiz olgu ile en çok yapılan cerrahi teknik olurken , patch (yama) plasti, greft uygulaması ve primer onarım, uygulanan diğer tekniklerdi. Penetran yaralanmak, genel durumu kötü ve sağ hemiplejisi olan bir olguda ise ligasyon uygulandı. Hastalar ortalama 14 günlük süre sonunda taburcu edildiler. Postoperatif dönemde endarterektomi yapılan bir olgumuz yoğun bakımda izlenirken akut gelişen miyokard infartüsü; penetran yaralanmalı diğer bir olgu ise postoperatif dördüncü gün muhtemelen hemorajik infarkt nedeniyle kaybedildi. Erken postoperatif mortalitemiz % 9.52 (iki olgu) olarak bulunmuştur. Sonuç: Karotid arter cerrahisi, özel korunma teknikleri gerektiren damar cerrahisi kompleks operasyonlarındandır. Klinik ve kişisel tecrübe mortalité ve morbiditeyi azaltan bir faktördür.

Abstract

Purpose: It is aimed to assess the morbidity and mortality rates of carotid surgery in traumatic and ac-quired carotid artery disease with this study. Patients and methods: Twenty-one patients who underwent carotid artery surgery in the Thoracic and Cardiovascular Surgery department of Medical School of Erciyes University between 1978 and 1997 were ret-rospectively reviewed . S eventeen (81 % ) of the patients were male and 4 (19 %) female. The ages of the patients were between 8 and 63 years (mean age 39.7). Results: The causes for operation were penetrating injury in 11 cases (523 %), arteriosclerotic lesions in nine cases ( 38.1 % ), and aneurysm due to Behçet's disease in one case (4.8 %). Endarterectomy was the most commonly used surgical technique (eight cases). Other techniques used were patch plasty, use of graft , and primary repair. Ligation was preferred in one case who had right hemiplegia and a poor general condition. The mean duration of hospitalization was 14 days. One case who underwent endarterectomy died in the postoperative period due to acute myocardial infarction and another who had a penetrating injury died probably due to a hemorrhagic cerebral infarction. The early postoperative mortality was 952% (two cases) Conclusion: Carotid artery surgery is a procedure during which special protection techniques are neccessary . We conclude that mortality and morbidity rates of this operation should decrease with increasing clinical and invidual experience.
