Özet
Amaç: Bu çalışmada aorta koartasyonu nedeni ile opere ettiğimiz olgularımızda operasyon teknikleri, komplikasyonları ve sonuçlarımızı literatür bulguları ile karşılaştırarak klinik tecrübelerimizi sunmayı amaçladık. Hastalar ve yöntem: Kliniğimizde 1.1 1996-1.12 1997 tarihleri arasında aorta koarktasyonu nedeniyle opere edilen sekiz olgu retrospektif olarak incelendi. Olguların hepsi erkek olup ortalama yaş 22 idi. Tüm olgularda posterolateral kesi ile cerrahi yaklaşım uygulandı. Dört olguda uç-uca anastomoz, bir olguda subklaviyan flep aortoplastisi, iki olguda peç plasti, bir olguda greft interpozisyonu uygulandı. Aort yetmezliği olan bir olguda koarktasyon onarımını takiben, aynı seansta median sternotomi ile aort valv replasmanı yapıldı. Bulgular: Bir olgu dışında tüm olgularda eşlik eden kardio-vasküler patoloji mevcuttu, ve bunların başında beş olgu ile patent duktus arteriosus gelmekteydi. Hastane ve geç dönem mortalitesi gözlenmedi. İki olguda geçici paradoks hipertansiyon oluştu. Ortalama hospitalizasyon süresi sekiz gündü. Sonuç: Bu sınırlı tecrübeler bize olguların yaş ve morfolojik yapısına göre uygun cerrahi tekniğin seçilmesi durumunda aorta toarktasyonu cerrahi tedavisinin etkili ve emin bir şekilde uygulanılabilirliğini göstermektedir.
Abstract
Purpose: In this study , our goal was to present comparative research of literature findings and clinical experience that have come from patients who were suffering from aortic coarctation in view of operative techniques, complications and the results of operation. Patients and methods: In our clinic between January 1, 1996 and December 1, 1997, there were eight cases of aortic coarctation that we examined retrospectively. All subjects were male with an average age of 22. In all cases postero-lateral surgical incision approach was applied. In four cases, end to end anastomosis; in one case, subclavian flap aortoplasty; in two cases, patch plasty; and in one, case greft interposition were performed. In one case, following the coarctational repair, aortic valve replacement was performed in the same session by using median sternotomy. Results: With the exception of one case, cardiovascular pathology were present in all cases whereas patent ductus arteriosus was the main disorder in five cases. There was no post operational and hospital mortality for all cases. In two cases there was temporary paradoxal hypertension. Average duration for hospitalization was eight days. Conclusion: Our limited experience has shown that when proper surgical techniques are chosen with regard to the age and the morphological structure of the cases, effective and reliable treatment can be established for aortic coarctation.
