Özet

Amaç: Katarakt cerrahisi sonrasında sütüre bağlı gelişebilen yüksek astigmatizma için güvenilir, etkin ve sonuçları önceden tahmin edilebilir bir tedavi protokolu belirlemek. Gereç ve Yöntem: Katarakt cerrahisi uygulanmış, üç ve üç diyoptrinin üzerinde kurala uygun astigmatizması olan 40 göz çalışma kapsamına alındı. Her olguda birden beşe kadar değişen sayıda sütür kesildi ve her sütür kesimi öncesi ve sonrası 2.,15.,30.,60. dakikalarda ve 1.haftada keratometre ile korneal astigmatizma ve akslar ölçüldü. Bulgular: Her sütür kesimini takip eden 1.haftanın sonunda astigmatizmada ortalama 3.86D azalma, aksta ise ortalama 20.7 derecelik bir değişiklik tespit edildi. Bir hafta sonunda meydana gelen toplam astigmatik azalmanın %83'ü ve aks değişikliklerinin %78'i sütür kesiminden sonra ilk 30 dakika içinde oluşurken, değişimlerin kalan kısmı ise ilk 30 dakikadan sonraki süre içinde meydana geldi. Sonuç: Katarakt cerrahisi sonrası oluşan yüksek korneal astigmatizmanın tedavisi için bir sütür kesildikten sonra eğer hala astigmatizma yüksek ise 30 dakika beklenip bir sonraki sütür de kesilebilir. Bu işlem aynı seansta, astigmatizma 3D altına ininceye kadar güvenli bir şekilde devam ettirilebilir.

Abstract

Purpose: The aim of our study was to determine a reliable, effective and predictable protocol for suture cutting to manage corneal astigmatism after cataract surgery. Material and Method: Selective suture cutting (from one to five sutures) was performed in 40 eyes with high with-the-rule astigmatism (³ 3 D) following cataract extraction. Corneal astigmatism and axis were measured by a keratometer before and 2 ,15 ,30 ,60 min. and one week after suture cutting. Results: An average of 3.86 D astigmatic correction was achieved for each suture cutting after one week following the procedure. The axis of astigmatism was found to shift an average of 20.7 degrees in each suture cutting at the end of one week. Most part of astigmatic (83%) and axis changes (78%) occurred within the first 30 minutes after suture cutting, but the remaining changes developed within one week. Conclusion: If high astigmatism still persists after first suture cutting following cataract surgery, it is not necessary to wait for a long period for cutting the remaining sutures and it is possible to cut following sutures after 30 minutes at the same session.
