Özet

Amaç:Bu çalışmada Erciyes Üniversitesi Tıp Fakiiltesi'nde 5.5.1994 tarihinden 13.8.2000 tarihine kadar renal transplantasyon uygulanan olguların mortalite ve morbidite ile ilgili verilerinin sunulması amaçlanmıştır. Gereç ve Yöntem: Sağkalım oranlarının hesaplanmasında Kaplan-Meier ve gruplardaki sağkalım oranlarının hesaplanmasında Log-rank testi kullanıldı. Bulgular: Düzenli hemodiyaliz veya SAPD uygulanan hastalardan transplantasyona engel oluşturacak kontrendi kasyonları olmayan 30 hastaya (Ortalama yaş 29.8 ±7.1 yıl) renal transplantasyon uygulandı. Donör kaynağı 19 (%63.3) olguda canlı, 11 olguda (%36.7) ise kadavra idi. Üç hastada akut rejeksiyon epizodu tanısı konarak tedavi verildi. Üç hastada kronik rejeksiyon, beş hastada cerrahi işleme bağlı komplikasyonlar gözlendi. Üriner infeksiyonlar en sık gözlenen infeksiyöz komplikasyondu. İzlem süresi boyunca dört hasta çeşitli sebeplere bağlı olarak kaybedildi. Greft kaybı gelişmiş altı hasta kronik hemodiyaliz programına geri döndü. Halen fonksiyone grefl ile takip edilen hasta sayısı 20 'dir. Sonuç: Bir yıllık hasta sağkahını % 96, beş yıllık hasta sağkalımı ise % 67 olarak tespit edilmiştir. Bir yıllık renal sağkalım %82, beş yıllık renal sağkalım ise % 48 olarak bulunmuştur. Canlı vericiden yapılan nakillerde 1 yıllık renal sağkalım oranı % 83, kadavra böbreği nakillerinde ise % 80 idi ve gruplar arasında anlamlı fark bulunamamıştır (p>0.05).

Abstract

Purpose: We aimed to evaluate the outcome of renal transplantation patients between the dales 5.5.1994 and 13.8.2000 at Erciyes University Medical Faculty. Material and Method: Survival rates were estimated by Kaplan-Meier test and survival rales in groups by Log-rank test. Results: A total of 30 patients without any contra-indications for transplantation underwent regular hemodialysis or continuous, ambulatory peritoneal dialysis and had renal transplantation. Nineteen of transplanted kidneys were received from living donors whereas cadaveric kidneys were used in 11 patients. Acute rejection was diagnosed and treated in 3 patients. Chronic rejection was diagnosed in 3 patients. Complications due to surgical procedure developed in 5 patients. Urinary infections were the most common form of infectious complications. A total of 4 patients died of various causes. Conclusion: Patients ' survival rates were 96% and 67% for the first year and for 5 years of the follow up period, respectively. On the other hand, graft survival rales were 82% and 48% for the first year and for 5 years of the follow up period, respectively. No significant difference was found between the graft survival rales of living and cadaveric donor kidneys (83% and 80%, respectively; p>0.005) for the first year.
