İdiyopatik hipogonadotropik hipogonadizmli hastalarda konvansiyonel gonadotropin tedavisinin lenfosit alt gurupları üzerine etkilerive IL-2 reseptör (IL-2R) ekspresyonu ile ilişkisi

Özet
Amaç: Seks steroidleri ve immün sistem ilişkisinin varlığı insanda bilinmekle birlikte, otoimmün hastalıkların patogenezini anlamak için gerekli olan bu ilişkinin mekanizması halen açık değildir. Erkeklerdeki major seks steroidlerinden olan testosteron ile IL-2R ekspresyonunun lenfosit düzeyindeki fonksiyonel ilişkisi insanda in-vivo ortamda henüz incelenmemiştir. Bu nedenlerden dolay idyopatik hipopogonadotropik hipogonadizmli (İHH) hastalarda planladığımız, konvansiyonel gonadotropin tedavisinin lenfosit alt gurupları ve periferal mononükleer hücrelerdeki (PMBC) IL-2R ekspresyonu üzerine etkileriyle ilgili araştırmanın ön çalışma sonuçlarını sunacağız. Gereç ve Yöntem: İHH tanısı alan 10 erkek hasta (ortalama yaş 25,6 ± 7,1) ve yaş/cinsiyet eşleştirilmiş 10 sağlıklı kontrol (ortalama yaş 27,1 ± 3,9) çalışmaya alınmıştır. Hastalar 12 ay boyunca haftada 3 gün; hCG ve human postmenopozal gonadotropin (hMG) ile tedavi edilmiştir. Lenfosit alt guruplarından CD3, CD4, CD8, CD16 ve CD19 yüzdeleri flow sitometri ile ölçülmüştür. Quantitative real-time PCR ile kontrollerin PMBC’lerinden elde edilen cDNA’larda IL-2R (hedef gen) ve beta-aktin (referans gen) ekspresyonu analiz edilmiştir. Abstract
Purpose: Although the presence of the interaction between sex steroids and the immune system is known, the mechanism of this interaction, which is very important in understanding the pathogenesis of the autoimmune disorders, is still unclear. In-vivo interaction of testosterone and IL-2R expression at lymphocyte level has not yet been investigated. Therefore, in the present study, we report the preliminary results of our ongoing study regarding the effects of conventional gonadotrophin therapy on lymphocyte subgroups and its relation with IL-2R expression in patients with idiopathic hypogonadotrophic hypogonadism (IHH). Materials and Method: Peripheral venous blood samples were obtained from 10 patients with IHH (Mean age 25.6 ± 7.1 years) and 10 age- and sex-matchedhealthy volunteers (Mean age 27.1 ± 3.9 years). Patients were treated for 12 months (3 days a week) with hCG and hMG. Lymphocyte subtype levels including CD3, CD4, CD8, CD16 and CD19 were analyzed by flow cytometry.
