Septic loosening due to brucella melitensis after bilateral knee prosthesis and two-stage total knee prosthesis revision

Özet

Total diz protezi sonrası görülen enfeksiyon ciddi bir komplikasyon olup, yüksek bir morbiditeye sahiptir. Brucella melitensis’in sebep olduğu prostetik eklem enfeksiyonu oldukça nadirdir. Buçalışmada bilateral çimentolu total diz protezi uygulanmasından sonra literatürde nadir olarak görülen her iki diz ekleminde septik gevşemeyle ortaya çıkan brusella enfeksiyonu olgusu ta-nımlanmıştır. Olgunun tanısı ateş, eklem şişliği gibi klinik bul-guların yanısıra, eklem aspirasyonu sonucu elde edilen eklem sıvısının incelenmesi ve labaratuvar bulgularıyla desteklenmiştir. Olguda hem direkt radyografiler, hem de kemik sintigrafileri septik gevşemeyi doğrulamıştır. Bu olguda antibiyotik tedavisini takiben her iki diz için revizyon total diz protezi uygulanmıştır. Revizyon için iki aşamalı cerrahi tercih edilmiştir. Birinci aşamada enfekte protezin çıkarılmasından sonra Teikoplanin içeren sement spacer yerleştirilmiş, ikinci aşama ise yirmi hafta sonrauygulanmıştır. Ameliyat sonrası 24 aya ulaşan takiplerinde herhangi bir komplikasyon görülmemiştir.

Abstract

Infection after total knee replacement is a serious complication associated with a high incidence of morbidity. A prostheticinfection due to Brucella melitensis is a very rare condition.In this study, we describe a case of brucellosis infection withseptic loosening of both knee joints after bilaterally cementedtotal knee prosthesis application; this is rarely encountered inthe literature. The patient diagnosis was established by clinicalfindings (fever, joint swelling, etc.) and supported by laboratoryfindings and analysis of joint fluid obtained from joint aspiration. Plain radiography and bone scintigraphy confirmed septicloosening. In this case, we performed total knee prosthesis revision bilaterally after antibiotic treatment. We preferred two-stagesurgery for the revision. In the first stage, the infected prosthesiswas removed and a Teicoplanin-loaded bone cement spacerwas inserted in both knees. Twenty weeks later, the secondstage was performed. During the 24-month follow-up period,no complications were seen.
