Analysis of distal radius fracture treatment with an external fixator

Özet

Çalışmaya eksternal fiksatör ile tedavi edilen kapalı distal radius kırıklı 37 olgu alındı. AO sınıflamasına göre yirmi sekiz olgu tip C, dokuz olgu tip B olarak değerlendirildi. Eksternal fiksatörlerinin çıkarılmasından sonraki on ikinci aylarında dorsifleksiyon, 64,3° (45-75°); supinasyon, 73,9° (62-85°); palmar fleksiyon, 74,2° (60-85°); ve pronasyon 84,1° (78-90°) olarak ölçüldü. Sarmiento Modifikasyonuna göre 24 olgu iyi, 8 olgu mükemmel, 5 olgu orta-kötü olarak değerlendirildi. Mayoskorlama sistemine göre ortalama değer 75,38 (55–90) olarakbulundu. Ortalama DASH skoru 8,27 idi. Toplam 12 olguda Refleks Sempatik Distrofi (RSD) meydana geldi. Bu olgulardan üçü medikal ve fizik tedaviye yanıt vermedi. Günlük yaşammemnuniyeti olmayan üç hastada bu RSD olan hastalardı. Toplam 19 olguda (%51) antibiyotik uygulama ile tedavi edilen yüzeyel çivi dibi enfeksiyonları gelişti. Hiçbir olguda derindoku infeksiyonu gelişmedi. Distal radius kırıklarında ekstenalfiksatör uygulamalarının cerrahi ve fonksiyonel sonuçlarınıntatminkar olduğu sonucuna vardık.

Abstract

This study included 37 patients with closed distal radius fractures treated with an external fixator. Twenty-eight fractureswere classified as type C and nine fractures were type B according to the AO classification system. Twelve months after theremoval of the external fixator, the dorsiflexion angle measured64.3° (45-75°), the supination angle measured 73.9° (62-85°),the palmar flexion angle measured 74.2° (60-85°) and the pronation angle measured 84.1° (78-90°). Twenty-four patientswere evaluated as good, eight patients were evaluated as excellent and five patients were evaluated as medium-poor according to the Sarmiento modification. The mean score was 75.38(55-90) according to the Mayo scoring system. The mean DASHscore was 8.27. In 12 patients, Reflex Sympathetic Dystrophy(RSD) was occurred. Three patients in this group did not respondto medical and physical therapy. Three patients without daily lifesatisfaction were RSD patients. In 19 patients (51%), superficialpin tract infection occurred and was treated with antibiotics.There were no deep tissue infections. We concluded that theapplication of an external fixator for distal radius fractures led tosatisfactory functional and surgical results.
