Özet

Rizomelik kondrodisplazi punktata (RCDP) ekstremitelerde proksimal kısalık, eklem kontraktürleri, vertebralarda koronalfissürler ve epifizlerde punktat kalsifikasyonlar ile tipik dismorfik yüz, bilateral katarakt, nöbetler, ciddi solunumsal sorunlar, ekzema, gelişme geriliği ve ağır mental retardasyonla karakterize otozomal resesif kalıtılan nadir görülen peroksizommetabolizma bozukluğu ile ilgili bir sendromdur. Tanı klinikve radyolojik bulgular ile konulmaktadır. Eritrosit plazmolojendüzeyleri, plazma fitanik asit düzeyleri ve plazma uzun zincirli yağ asit düzeyleri gibi peroksizomal fonksiyonlar RCDP tanısının biyokimyasal belirteçleridir. Genetik danışmanlık önerilerek prenatal tanı yöntemleriyle tekrarlar önlenebilir. Bu çalışmada ekstremitelerin proksimalinde kısalık, epifizlerde noktavikalsifikasyon bozukluğu, vertebralarda koronal fissür, kataraktve hipotonisi saptanan, izlemin ikinci ayında araya giren altsolunum yolu enfeksiyonunu takiben eksitus olan bir RCDP olgusu seyrek görülmesi nedeni ile sunuldu.

Abstract

Rhizomelic Chondrodysplasia Punctata (RCDP) is a rare autosomal recessive syndrome characterised by punctuate calcifications of the cartilage associated with proximal limb shortening,joint contractures, vertebral coronal clefts, typical dysmorphicfacies, bilateral cataracts, seizures, serious respiratory problems,failure to thrive and severe mental-motor retardation. Diagnosisis usually made based on clinical and radiological criteria. Peroxisome functions, such as the red blood cell concentration ofplasmalogenes and the plasma concentrations of phytanic acidand very long chain fatty acids are biochemical indicators ofRCDP Recurrences can be avoided through genetic counselling..In this article, we present a case of the rare disorder RCDP manifested as proximal limb shortening, punctuate calcifications ofthe cartilage, vertebral coronal clefts, cataracts and hypotonia.The patient died in the second month of follow-up due to inter-current lower respiratory tract infection.
