Herlyn-Werner-Wunderlich syndrome with term pregnancy: The presentation of a rare case

Özet
Herlyn-Werner-Wunderlich sendromu uterus didelfis, kör vajina ve ipsilateral renal agenezinin birlikte görüldüğü nadir bir durumdur. Genellikle menarş sonrası başlayan remittan pelvikağrı ve hematokolpos nedeniyle ele gelen kitle şikayetine sebep olur. Bu yazıda uterus didelfis, kör vajina ve aynı taraflı renal agenez saptanan term gebe 21 yaşında kadın hasta sunuldu. Hastanın gebeliği ultrasonografik ölçümlere göre 35haftalık idi. Fetal distres saptanması üzerine yapılan acil sezeryan girişiminde solda fetüsün bulunduğu uterus, sağda ise yaklaşık 20 hafta cesametinde diğer uterus gözlemlendi. Sağ uterustaki serviksten ilerletilen ring forsepsin ucunun vajinadan görülmedi belirlendi. Operasyondan sonra çekilen abdominaltomografide sağda renal agenez saptandı. Tanısı genellikle menarş sonrası konulan Herlyn-Werner-Wunderlich sendromuna,sunulan olguda olduğu gibi sezeryan sırasında da rastlanabilir.
Abstract
Uterus didelphys with obstructed hemivagina and ipsilateralrenal agenesis is a rare entity referred to as Herlyn-Werner-Wunderlich syndrome. It usually presents after menarche withremittent pelvic pain and a palpable pelvic mass due to hematocolpos. In this article, we present a 21-year-old patient witha term pregnancy presenting with uterus didelphys, obstructedhemivagina, hematocolpos, and ipsilateral renal agenesis. Anemergency cesarean section was performed because of fetaldistress. The fetus was in the left uterus, since due to hematocolpos, the right uterus was the size of 20 weeks of pregnancy.After delivery, the tip of ring forceps placed through the rightcervix was not seen in the vagina. Right renal agenesis wasdiagnosed by abdominal CT taken after the cesarean section.Herlyn-Werner-Wunderlich syndrome is usually diagnosed after menarche, but in this case, it was diagnosed during a cae-sarean section.
