Solitary cecal diverticulitis mimicking malignancy: Case report and review of the literature

Özet

Çekum divertikülleri, gastrointestinal sistemin gerçek divertikülleridir. Genellikle asemptomatik olmakla birlikte ancakkomplike olduğu zamanlarda klinik belirti verirler. En sık rastlanan komplikasyonu divertikülittir. Vakaların yarısı çekumun ön duvarına yerleşir ve bu yerleşimi nedeniyle sıklıkla akut apandisiti taklit ederler. Divertikül şayet çekumun arka duvarında yerleşmiş ise maligniteyi takit eden inflamatuvar bir kitle oluşturur. Bu yazıda maligniteyi taklit eden çekal divertiküllitli bir olgu sunuldu. Akut apandisit tanısı ile opere edilen olguda apandiksin olağan gözlenmesi ve çekumun karın arka duvarına yapışık ele sert gelen bir kitlenin bulunması üzerine çekumtümörü düşünülerek işlemi sonlandırılan olgunun kolonosko-pik incelemede, çekumun posteromedial duvarında divertiküle işaret eden orifis saptandı. Soliter çekum divertiküliti tanısı ileantibiyotik tedavisi alan olguya, taburcu edildikten sonra aynışikâyetlerle tekrar başvurması üzerine sağ hemikolektomi uygulandı. Cerrahi piyesin histopatolojik incelemesinde tümoral oluşuma rastlanmadı. Sağ alt kadranda ağrı veya kitle ile gelen hastalarda Soliter çekum divertikülü de ayırıcı tanı da akla gelmelidir.

Abstract

Solitary cecal diverticulum is one of the true diverticula in thegastrointestinal tract. It is usually asymptomatic; however, clinical findings come forward when it is complicated. The mostcommon complication is diverticulitis. Solitary cecal diverticulum involves the anterior wall of the cecum in one half of casesand often mimics acute appendicitis because of this placement.If the diverticulum settles at the posterior wall of the cecum, aninflammatory mass mimicking malignancy could form. In thisarticle, we present a case presenting solitary cecal diverticulitismimicking malignancy. Orifice pointing on the solitary cecaldiverticulum was determined during a colonoscopy of the patient, who received a diagnosis of appendicitis. However, thesurgical procedure was terminated due to regular observationof the appendix in the patient despite a diagnosis of acute appendicitis; a mass was found in the posterior of the cecum, at- tached to the abdominal wall. Upon persistence of the samplecomplaint after hospital discharge, a right hemicolectomy wasperformed on the patient with a diagnosis of and treatment forsolitary cecal diverticulitis. A solitary cecal diverticulitis patientwith a diagnosis of antibiotic therapy, right hemicolectomy wasperformed at the request of the same complaints again. Theformation of a tumor was not observed in the histopathological examination of the specimen. Solitary cecal diverticulitisshould be considered in the differential diagnosis of patientspresenting with right lower quadrant pain and a mass.
