A case of renal mucinous tubular and spindle cell carcinoma

Özet

Müsinöz tübüler ve iğsi hücreli karsinom yeni tanımlanmış, nadir bir renal hücreli karsinom tipidir. Genellikle böbrekte sınırlı olup düşük malign potansiyele sahiptir. Tümör sıkı tübüler mimari, fokal papiller yapılar, müsin üretimi ve köpüksü hücrelere ek olarak iğsi hücre morfolojisiyle öncelikle sarkomatoid papiller renal hücreli karsinom olmak üzere diğer renal hücreli karsinomlardan ayrılmalıdır. Bu yazıda hematüri şikayeti ile başvuran 68 yaşında erkek olgu sunuldu. Batın ultrasonografik incelemede sağ böbrek orta ve üst polünü işgal eden 8 cm çaplı heterojen iç yapılı solid bir kitle saptandı. BT’sinde sağ böbrek üst kutbunda izlenen kitle nefrektomi ile çıkartıldı. Patolojik incelemede miksoid stromada, nükleer atipi içermeyenküboidal hücreler ile döşeli değişik boyutlarda tübüller, kordyapıları ve az sayıda iğsi hücrelerden oluşan patolojik dokununimmünohistokimyasal olarak vimentin, sitokeratin 7 ve sitokeratin 19 diffüz kuvvetli, EMA fokal kuvvetli, yüksek molekül ağırlıklı sitokeratin fokal pozitif reaksiyon verdiği belirlendi. Bu bulgularla Renal Müsinöz Tübüler ve İğsi Hücreli Karsinom tanısı alan olgu 24 aydır komplikasyonsuz izlenmektedir.

Abstract

Mucinous tubular and spindle cell carcinoma is a newly-defined, rare type of renal cell carcinoma. It is usually self-limited in the kidney and has a low malignancy potential. Thetumour should be differentially diagnosed from other renal- cell carcinoma, primarily the sarcomatoid papillary renal-cellcarcinoma, with its dense tubular architecture, focal papillarystructures, mucinous production and spindle cell morphology in addition to the foamy cells. In this article, a 68 year-oldmale with a complaint of hematuria was presented. During theultrasonographic abdominal examination, an 8-cm diametersolid mass which has a viable heterogenous internal structurewas detected occupying the middle and upper pole of the rightkidney. The mass observed in the upper pole of the right kidney during the BT was removed with nephrectomy. During thepathological examination, in the myxoid stroma, the pathological tissue which consisted of cuboidal cells which excludenuclear atypia and lined various-sized tubules, cord structuresand a few spindle cells, was defined to give a reaction immunohistochemically to vimentin, cytoceratine 7 and cytoceratine 19 diffuse strong, EMA focal strong, high molecular weightcytoceratine focal positive, respectively. The case, diagnosedas Renal Mucinous Tubular and Spindle Cell Carcinoma withthese findings, has been monitored without any complicationfor 24 months.
