The impact of the use of neuronavigation together with intraoperative ultrasonography in minimally invasive intracranial cavernous hemangioma surgery

Özet

Amaç: Mortalite ve morbiditeyi azaltmak için kavernöz hemanjiyomun cerrahi tedavisinde intraoperatif ultrasonografi ile birlikte nöronavigasyon kullanımının katkısını araştırmak. Materyal ve Metot: Erciyes Üniversitesi Nöroşirürji Departmanında Haziran 2000 ve Eylül 2010 arasında nöronavigasyon ve intraoperatif ultrason rehberliğinde ameliyat edilen kavernöz hemanjiyomu olan toplam 73 olgu retrospektif olarak incelendi. Olgular cerrahi nedeniyle nörolojik kötüleşme açısından değerlendirildi. Bulgular: Lezyonlar çoğunlukla, özellikle temporal bölgede, supratentorial olarak yerleşikti. Supratentorial yerleşimli lezyonların en sık şikayeti epilepsi idi. Kanama, baş ağrısı ve fokal nörolojik defisit gibi belirti ve bulgular çoğunlukla infratentoryal bölgedeki lezyonlarda gözlendi; 73 vakada toplamda 80 kavernöz hemanjiom çıkarıldı. Olguların 3'ünde, nörolojik defisitlerde geçici bir artış gözlendi; ancak, geç dönemlerde, cerrahiye bağlı herhangi bir kalıcı morbidite veya mortalite gözlenmedi. Sonuç: İntraoperatif ultrason ile birlikte nöronavigasyon kullanılarak, beyin omurilik sıvısı drenajı nedeniyle beynin yer değiştirmesi sonucu lezyonun navigasyon görüntüsü ile gerçek yerleşim arasındaki uyumsuzluk restore edilebilir; böylece morbidite ve mortalite azaltılabilir.

Abstract

Objective: To investigate the contribution of the use of neuronavigation together with intraoperative ultrasonography in thesurgical treatment of cavernous hemangiomas to the decrease of mortality and morbidity.Materials and Methods: A total of 73 cases with cavernous hemangiomas, operated on under the guidance of neuronavigationand intraoperative ultrasonography between June 2000 and September 2010 in the Neurosurgery Department of the Universityof Erciyes, were investigated retrospectively. The cases were evaluated from the point of neurological deterioration due to surgery.Results: The lesions were mostly located supratentorially, especially in the temporal region. The most common complaint ofsupratentorially located lesions was epilepsy. Such fndings and symptoms as bleeding, headache, and focal neurological defcitswere observed mostly in the lesions of infratentorial region; 80 cavernous hemangiomas in 73 cases were removed in total. In 3of the cases, a temporary increase in the neurological defcits was observed; however, in the late periods, no persistent morbidityor mortality due to surgery was observed.Conclusion: By using neuronavigation together with intraoperative ultrasonography, the inconsistency between real locationand the navigation image of the lesion resulting from the brain shift due to cerebrospinal fuid drainage can be restored; so, themorbidity and mortality can be decreased.
