GIANT ANGIOSARCOMA OF THE TONGUE CAUSING SPONTANEOUS BLEEDING AND UPPER AIRWAY OBSTRUCTION

Özet

Dil anjiosarkomları oral kavitenin nadir mezankimal tümörleridir. Cerrahi, kemoterapi ve radyoterapi gibi tedavileri olmasına rağmen, agresif tümörlerdir. Nadir görülmesinden dolayı henüz kabul edilmiş bir tedavi algoritmi yoktur. Bu olgu sunumunda spontan kanama ve solunum sıkıntısı ile gelen ve kanama için eksternal karotid arter ligasyonu; solunum sıkıntısı için ise trakeotomi açılan devasa ilk dil anjiosarkomu sunulmaktadır. Bu olgu sunumunda daha önceki dil angiosarkomları da detaylı bir şekilde sunulmuştur. Cerrahi sırasında ciddi kanama riskinden dolayı cerrahlara kartotid arter ligasyonu önerilmektedir.

Abstract

Tongue angiosarcomas (TAS) are rarely publishedmesenchymal tumors of the oral cavity. Despitesuitable treatment options such as surgery,chemotherapy and radiotherapy, they are aggressivelethal tumors. There is no globally accepted treatmentalgorithm for TAS due to scarcity of case reports. Inthis case report; we presented the first case of giantTAS presenting with both spontaneous bleeding andrespiratory distress needing tracheotomy for upperairway obstruction and external carotid artery ligationfor bleeding control. With this case report, a detailedreview of previous TAS cases was also presented. Therisk of abundant bleeding during surgical interventionand need for appropriate arrangements for externalcarotid artery ligation was emphasized for thesurgeons.
