Seks hormon bağlayıcı globulin ile tiroid ve seks hormonları arasındaki ilişki

Özet

Karaciğer kökenli bir glikoprotein olan seks hormon bağlayıcı globülin (SHBG):, plazma steroid hormonlarına yüksek bağlanma afinitesine sahip olup; bu etkisi ile de özellikle seks hormonlarının vücûttaki fonksiyonları üzerine önemli etkileri mevcuttur. Seks hormonlarının fonksiyonlarını oldukça etkileyen SHBG' in plazma düzeyini birçok faktör etkileyebilir. Bunlar arasında eksojen testosteron artması, sistemik hastalıklar, ilaç kullanımları sayılmaktadır. Bu karşılıklı etkileşimler aynı zamanda vücut yağ oranı üzerine de oldukça önemli değişiklikler yapmaktadır. Özellikle kadınlarda artmış androjenite visceral yağ oranını arttırırken, kilo kaybı ve androjenitede azalma yağ depolarını normal sınırlara çekebilmektedir. Çalışmamızda 1998 yılında Cerrahpaşa Tıp Fakültesi İç Hastalıkları Ana Bilim Dalı Endokrinoloji Bilim Dalı'na baş vuran 22 hastanın (19 kadın, 3 erkek) bel/kalça oranları, SHBG, T3, T4, TSH, tiroid hormon bağlayıcı globülin (TBG), testosteron, serbest testosteron, estradiol, DHE-ASO4 değerleri ve karşılıklı etkileşimleri incelenmiştir. Hastaların verileri Pearson korelasyon testleri ile karşılaştırıldığında; DHEASO4 ile estradiol, serbest testosteron ile T3, T4 ve testosteron; SHBG ile T3, T4 ve testosteron; T3 ile T4; ve T3 ve T4 ile testosteron arasında anlamlı ilişki olduğu görülmüştür. Böylece çalışmamızda SHBG ile beklendiği gibi TBG arasında anlamlı bir ilişki olmadığı, bunun yanında SHBG ile tiroid hormonları ve testosteron arasında anlamlı bir ilişki olduğu gösterilmiştir. Tiroid hormonlarının yüksek saptandığı vakalarda SHBG ve testosteron düzeyleri yüksek, serbest testosteron normal düzeylerde bulunmuştur. Bu da testosterondaki yükselmenin SHBG' deki yükselmeye bağlı olduğunu düşündürmektedir

Abstract

Sex hormone-binding globulin (SHBG): a glycoprotein which is derived from the liver, has higher affinity for binding plasma steroid hormones and has great effects on the functions of these hormones. Plasma SHBG consent- ration is effected by different factors such as increase in free testosterone levels, systemic diseases and drug intake. Body fat distribution is also affected by these changes. In women androgenity increases the viseral fat weight loss and a decline in androgenity normalize plasma fats. In our study 22 patients (19 female and 3 male) were observed for their waist/hip ratios, SHBG, T3, T4, TSH, thyroid binding globulin (TBG), testosterone, free testosterone, estradiol, DHESO4 values and their relationships between them. Using Pearson's correlation tests, there are significant relationship between DHEASO4 and estradiol, free testosteron and T3, T4 and testosterone; SHBG with T3, T4 and testosterone; T3 with T4 and testosterone and between T4 and testosterone. In our study we tried to find a relationship between SHBG and TBG, but despite the current literature, found none. We also show that there is a significant relationship between SHBG and thyroid hormones and testosterone levels. In patients who had higher levels of thyroid hormones; there is also an increase in serum levels of SHBG and testosterone, but serum levels of free testosterone were found to be normal. We conclude that the increase in testosterone levels in patients with high levels of thyroid hormones is the result of high levels of SHBG in these patients.
