Diabetes mellitus olgularında kardiyovasküler otonom nöropatinin ve sol ventrikül fonksiyonlarının incelenmesi

Özet
Diabetes mellituslu hastalarda kardiyovasküler otonom nöropati (KON) ve sol ventrikül disfonksiyonu sık görülen komplikasyonlar olup mortalite ve morbiditeyi önemli ölçüde artırmaktadır. Çalışmamızda, insulin bağımlı olmayan kardiyovasküler yönden asemptomatik diabetes mellituslu hastalarda KON tutulumunu ve sol ventrikül fonksiyonlarını incelemeyi amaçladık. Araştırmamızı üç grup üzerinde yaptık. Birinci grup diabet yaşı 0-10 yıl olan 30 hastadan, ikinci grup diabet yaşı 10 yıl üzeri olan 30 hastadan, üçüncü grup (kontrol grubu) 20 sağlıklı kişiden oluşmaktaydı. Tüm olgulara KON tutulumu olup olmadığını anlamak için kardiyovasküler otonom fonksiyon testleri yapıldı. Ayrıca radyonüklid ventrikülografi yöntemi ile tüm olguların sol ventrikül fonksiyonları incelendi. Çeşitli derecelerde KON tutulumu olanların oranı birinci grupta % 60, ikinci grupta ise %90'dı. Sol ventrikül sistolik disfonksiyon oranı birinci grupta %10, ikinci grupta %13 iken diyastolik disfonksiyon oranı birinci grupta %30, ikinci grupta %47 idi. Diabet gruplarında KON tutulumu ve sol ventrikül diyastolik disfonksiyonunun oldukça sık görüldüğü izlendi ve sol ventrikül fonksiyonlarının incelenmesinde radyonüklid ventrikülografinin uygun bir yöntem olduğu kanısına varıldı
Abstract
Cardiovascular autonomic system neuropathy (CAN) and left ventricular dysfunction are often seen complications in diabetic patients, and increase mortality and morbidity. The aim of this study was to observe the functions of the left ventricular and CAN in patients with noninsulin dependent diabetes mellitus. The subjects did not have any cardiovascular symptom. There are three groups in our study. In the first group there are 30 patients, anamnestic diabetic ages between 0-10 years old. The second group, 30 patients whose diabetic age is over 10 years. In the control group there are 20 healthy people. Cardiovascular autonomic system function tests was prepared to the all subjects, and the functions of left ventricular is investigated by radionuclide ventriculography (RNV). The ratio of the patients with CAN was %60 in first group and %90 in second group. On the other hand, the ratio of the patients with systolic dysfunction was %10 in first group, %13 in second group and ratio of diastolic dysfunction was %30 in first group, %47 in second group. As a result it's concluded that CAN and left ventricular dysfunction are often seen in diabetic patients, RNV is a suitable method to evaluate left ventricular functions.
