Efficacy of acarbose in the impaired glucose tolerance

Özet

Çalışmamızda glukozidaz inhibitörü acarbose'nin oral glukoz tolerans testi (OGTT) ile diabetes mellitus (DM) veya glukoz tolerans bozukluğu (GTB) saptanan hastalarda etkisini değerlendirdik. Ortalama yaşı 60.2±10.9 yıl, vücut kitle indeksi (BMI) 27.5±5.3 kg/m2 olan ve daha önce anti-diabetik tedavi uygulanmamış 56 olguya glisemi düzeylerini etkileyecek ajanlar verilmeden, 3 ay 1400-1600 kcal'lik diyet uygulandı. Tekrarlanan OGTT' de DM saptanan 42 ve GTB saptanan 14 hastanın başlangıç, 60. ve 120. dakika glisemi düzeyleri ilk OGTT' den anlamlı farklılık göstermedi. Tedaviyi etkileyecek ajanlar verilmeden yemekle birlikte ilk 15 gün 3x50 mg, sonraki 4 ay 3x100 mg Acarbose verildi ve önceki diyetlerine devamı önerildi. Diabetik 42 hastanın 22'si çalışma sonunda hala diabetik olup 4'ünde açlık glisemi düzeyleri yüksek kaldığı halde 60. ve 120. dakika glisemi düzeyleri normale döndü. GTB ve normal eğri 10'ar olguda saptandı. Abstract

We evaluated the effects of α-glucosidase inhibitor Acarbose in patients with diabetes mellitus (DM) or impaired glucose tolerance (IGT) diagnosed according to oral glucose tolerance test (OGTT). 56 patients with a mean age of 60.2±10.9 years, body mass index (BMI) of 27.5±5.3 kg/m2 were enrolled into the study. They were not given any antidiabetic treatment before. All the patients were given a diet of 1400-1600 kcal for 3 months. When OGTT was repeated, 42 patients with DM and 14 patients with IGT had not any significant difference in the 1st, 60th and 120th minute glycemia levels. Patients were given Acarbose 50 mg tid for the first 15 day and then 100 mg tid for 4 months with the same diet and without any treatment altering agent. 22 diabetic patients out of 42 were still diabetic at the end of study, but in 4 cases although high fasting glycemia levels, 60th and 120tth minute glycemia levels were normalised.
