Özet

İnce iğne aspirasyon sitolojisi (HAS) tiroid nodüllerinin operasyon öncesi değerlendirilmesi ve klinik yönetiminde kolay, hızlı ve düşük maliyetli bir yöntemdir, ince iğne aspirasyon sitolojisinde duyarlılık ve özgüllükle ilişkili %40-99 arasında değişen farklı rakamlar bildirilmiştir. Biz yaklaşık 3 yıllık bir süreçte HAS yapılan 1134 tiroid nodüllü olguda HAS sonuçları ile kesin patoloji sonuçları karşılaştırılarak yöntemin duyarlılığı ve özgüllüğünü değerlendirdik, incelenen 1134 llAS'den 286'ı (%25.2) değerlendirme için uygun özellikte bulunmadı. Tanı alan 848 (%75.8) olguda sitopatolojik tanılar 4 (benign, malign, şüpheli sitoloji ve folliküler neoplazi) ana başlıkta toplandı.Bizim hastanemizde II-AS yapılan hastalardan 21.5'ne tirodektomi uygulanmıştır.Bunların 30'u sitoloji de yetersiz tanısı alan gruba aittir. Biz serimizde duyarlılığı: %85, özgüllüğü: %94, pozitif prediktif değeri %71, negatif prediktif değeri %97 olarak saptadık. Kendi serimizde doğruluk oranı %92'dir. Bu değerler llAS'nin tiroid nodüllerinin yönetiminde etkili bir yöntem olduğunu desteklemektedir.

Abstract

Fine needle aspiration biopsy (FNAB) is an easy, cheap and a fast procedure for the preoperative evaluation and management of tyroid nodules. The specifity and sensitivity of FNAB has been reported to be between 60-95%. We assessed the sensitivity and specifity of the procedure by comparing the FNAB results to absolute pathologic results on 1134 patients for a period of three years. Out of 1134 FNAB results, 286 were excluded, because of not being sufficient for evaluation (%25.2). The 848 (%75.8) cases where a diagnose was made were divided into 4 main groups (Benign, malignant, suspicious cytology, and follicular neoplasm). In our center tyroidectomy was performed to 215 patients undergoing FNAB. 30 of them belonged to the group of excluded for insufficient biopsy material. In our series we calculated the sensitivity as %85, the specifity as %94, the positive predictive value as %71,and negative predictive value as %97. In our study the rate of truth is %92. These values support FNAB to be an effective method for the management of tyroid nodules.
