Özet

Klinik hipokalsemi, tiroidektomi komplikasyonlarının en sık rastlanılanlarından biridir. Çalışmamızda, parathormon (PTH) tayininin postoperatif hipokalsemi gelişecek hastaları öngörmedeki etkinliğini araştırmayı amaçladık. Tiroid hastalıkları nedeniyle total veya totale yakın tiroidektomi uygulanan 40 hasta incelendi. Hastalara preoperatif dönemde ve postoperatif 1, 6, 18, 30, 42. saatlerde ve birinci haftada kalsiyum tayini, preoperatif, postoperatif birinci saat, 6. saat ve 7. gün parathormon tayini yapıldı. Altı hastada (%15) klinik hipokalsemi gelişti. Klinik hipokalsemiyi öngörmede kan kalsiyum seviyesi ancak 18. saatten sonra etkili bulunurken, parathormon tayinlerinin postoperatif birinci saatten itibaren tüm ölçümlerde etkili olduğu tespit edildi. Abstract

Clinical hypocalcemia is among the frequent complications after thyroidectomy. In this study, we aimed to evaluate the value of parathyroid hormone (PTH) levels to predict postoperative hypocalcemia. Fourty patients who underwent totaly or near totaly thyroidectomy for thyroid disorders were analysed by surveying the calcium levels preoperatively, and on postoperative hours 1., 6., 18., 30., 42. and at postoperative a week. PTH levels were evaluated 1 hour preoperatively and postoperatively, 6th. hour and at day 7 postoperatively. 6 patients (15%) developed clinical hypocalcemia. Serum calcium levels were predictive for clinical hypocalcemia only after 18 hours postoperatively, whereas PTH levels were effectively predictive in all samples starting after the first hour postoperatively (p
