Tip 1 ve tip 2 diyabetik hastalarda hemostatik parametrelerin değerlendirilmesi ve diyabetik vasküler komplikasyonlar ile ilişkisi

Özet
Diabetes mellitusta trombofiliye neden olan pıhtılaşma anormallikleri olduğu bilinmektedir. Kötü metabolik kontrol ile ilişkili görünen trombofilinin mikro- ve makrovasküler hastalığa yolaçabileceği düşünülmektedir. Bu çalışmanın amacı, diabetes mellituslu hastalarda metabolik kontrol, kan lipidleri ve diyabetik komplikasyonları gözönüne alarak hemostatik sistem faktörlerini incelemektir. Çalışma grubu, 48 tip 1 diyabetik (yaş: 27.93±1.06 yıl), 22 tip 2 diyabetik hastadan (yaş: 54.41±1.72 yıl) ve 47 sağlıklı kişiden (yaş: 27.89±1.24 yıl) oluşturuldu. Fibrinojen, pıhtılaşma faktörü II, V, VII, VIII, IX, X, protein C, protein S ve antitrombin III düzeyleri çalışıldı. HbA1c düzeyleri hem tip 1 ve hem tip 2 diyabetik hastalarda yüksekti (sırasıyla %9.31±0.30 ve %8.28±1.87). Antitrombin III, protein C, protein S ve faktör VIII düzeyleri gruplar arasında farklı saptanmadı. Tip 1 diabetes mellituslu hastalarda, faktör V, VII, IX ve X düzeyleri sağlıklı bireylerle karşılaştırıldığında belirgin yüksek tespit edildi. Fibrinojen düzeyi diyabetik komplikasyonu olan ve olmayan tip 1 ve tip 2 diyabetik hastalarda yüksek bulundu. Faktör II düzeyi, tip 2 diyabetik hastalarda kontrollere göre düşük saptandı. Faktör V ve VII'nin trigliserid düzeyleri ile, faktör X ve antitrombin III'ün total kolesterol düzeyleri ile ilişkili olduğu bulundu. Otonomik nöropatisi olan hastalarda protein C düzeyleri düşük, periferik nöropatisi olan hastalarda faktör X düzeyleri belirgin yüksek tespit edildi. Bu hemostatik faktörlerin metabolik kontrol ile bir ilişkisi gözlenmedi. Diğer faktörler ile mikro- ve makrovasküler komplikasyonlar arasında ilişki saptanmadı.
Abstract
Coagulation abnormalities occur in the course of diabetes mellitus, resulting in a state of thrombophilia. A hypercoagulable state associated with metabolic disturbance may contribute to micro- and macrovascular disease. The aim of this study was to evaluate factors of hemostatic system in type 1 and type 2 diabetes mellitus patients taking into account metabolic control, blood lipids and diabetic complications. The study population consisted of 48 patients with type 1 diabetes (age: 27.93±1.06 years), 22 patients with type 2 diabetes (age: 54.41±1.72 years) and 47 healthy subjects (age: 27.89±1.24 years). The plasma levels of fibrinogen, coagulation factors II, V, VII, VIII, IX, X, protein C, protein S, and antithrombin III were determined. HbA1c levels were high both in type 1 and 2 diabetic patients (9.31±0.30 and 8.28±1.87% respectively). There was no significant difference in antithrombin III, protein C, protein S, and factor VIII levels between the groups. The mean factor V, VII, IX and X levels in patients with type 1 diabetes mellitus were elevated markedly compared with healthy subjects. Fibrinogen level was significantly increased in both type 1 and 2 diabetic patients with or without diabetic complications. Factor II level was found to be low in patients with type 2 diabetes compared with healthy subjects. Factor V and VII levels were correlated with triglyceride levels and factor X, antithrombin III with total cholestrol levels. Protein C levels were significantly low in diabetic patients with autonomic neuropathy. Factor X levels were high in patients with peripheric neuropathy. There was no significant correlation of these hemostatic factors with metabolic control. The analysis shows no significant difference between the groups. Other factors were not found to be linked with micro- and macrovascular complications.
