Özet

Tiroid mikropapiller kanserinin ilk ve tek bulgusu servikal lenf nodu metastazı olabilir. Papiller mikrokarsinomun başlangıç belirtisi boyunun lateralinde kitle olan beş hastayı sunduk. Olguların 1'i (%20) erkek, 4'ü (%80) kadın olup yaş ortalaması 33.6 yıl (28-42) idi. Hastaların şikayeti boyunun lateralinde ağrısız, haraketli ve yuvarlak kitle idi. Ultrasonda servikal kitlelerin lenf nodları olduğu gösterildi. Hastaların tümünde fizik incelemede tiroid bezinde nodül palpe edilmedi. Bulgular: Tüm hastalarda mikropapiller kanser tanısı preoperatif dönemde yapılan ince iğne aspirasyon biyopsisi ve lenf nodlarının exsizyonel biyopsisi ile kondu. Hastalara tiroidektomi, konservatif boyun diseksiyonu ve takibinde radyoaktif iyod tedavisi uygulandı. Ortalama 2.2 yıl takip edilen hastalardan ölen olmadı, nüks ve metastaz saptanmadı. Soliter servikal lenf nodlu genç hastalarda, papiller tiroid mikrokarsinomundan lenf nodu metastazı da göz önünde bulundurulmalıdır.

Abstract

The first and sole manifestation of papillary thyroid microcarcinoma may be cervical lymph node metastases. In the five patients here reported, a mass in the lateral aspect of the neck was the initial symptom of the papillary microcarcinoma. There were one man and four women, mean age 33.6 years (28-42), who complained of round, movable, painless masses in the lateral aspect of the neck. Cervical lymph node was demonstrated by neck ultrasound. In all cases, thyroid nodules were not palpable on physical examination.The diagnosis was made preoperatively by fine-needle aspiration cytology and excision biopsies of the nodes in cases. All patients underwent thyroidectomy with conservative neck dissection follewed by radioactive iodine therapy. After a follow-up means 2.2 years, all patients are alive with no apparent signs of recurrence or metastasis.In a young patients with solitary lateral cervical lymph nodes, the diagnosis of lymph node metastasis from papillary thyroid microcarcinoma should be considered.
