Özet
Tiroidin benign hastalıklarında hastalığın tanısı ve tiroid hormon profili tedavi yaklaşımını belirler. Hipertiroidi varlığının cerrahi tedavi komplikasyonlarını artırdığı konusunda hala görüş birliği yoktur. 2006-2008 yılları arasında benign patolojiler nedeni ile aynı ekip tarafından tiroidektomi yapılan 201 olgu, hipertiroidi varlığına göre iki gruba ayrılarak, operasyon seçiminin, demografik dağılım, postoperatif mobidite ve mortaliteye olan etkilerinin değerlendirilmesi amaçlanmıştır. Ötiroid olan 145 hasta ile hipertiroidisi bulunan 56 hastanın yaş cinsiyet dağılımları benzerdi. Ötiroid grupta hastaların 129 (%89)'unu, hipertiroidili hastaların ise 44 (%78)'ünü multinodüler guatrlı hastalar oluşturmaktaydı. Ötiroid hastaların 112 (%77)'sine ve hipertiroid olanların 48 (%86)'ine total tiroidektomi yapılırken, soliter adenomlu hastalara genellikle hemitiroidektomi, diğer hastalara da değişik cerrahi girişimler uygulandı. Ameliyat sonrası ötiroid grupta 19 (%12), hipertiroid grupta 6 (%11) hastada komplikasyon gelişti, en sık görülen komplikasyon ise hipokalsemi idi. Gruplar arasında operatif tedavi yöntemleri, komplikasyonlar ve hastanede kalış süresi açısından farklılık saptanamadı. Sonuç olarak hipertiroidi varlığı uygun hazırlık yapılıp ve özenli teknik uygulandığında cerrahi tedaviye ek bir olumsuzluk yaratmaz.
Abstract
For benign thyroid diseases, diagnosis of the disease and thyroid hormone profile, determines the treatment approach. Accurate diagnosis and evaluation of thyroid hormone profile determines the treatment approach for benign thyroid diseases. There is still some debate over whether hyperthyroidism elevates the surgical complication rates or not. Present study evaluates 201 patients who underwent operation for benign thyroid diseases and divided into two groups status of hyperthyroidism between 2006-2008 by the same team, demographic distrubition, operation type, postoperative morbidity and mortality rate between two groups were evaluated. Hundered and forty-five patients were euthyroid and 56 patients were hyperthyroid, which had similar distributions. 129 patients (89%) of the euthyroid group and 44 patients (78%) of the hyperthyroid group had multinoduler goitre. Hundered and twelve patients (77%) from the euthyroid group and 48 patients (86%) from the hyperthyroid group, underwent total thyroidectomy whereas in patients with soliter adenoma hemithyroidectomy was the precedure of choice. Remining patients, underwent various procedures. Complications occured in 19 patients from the euthyroid group (12%), and in 6 patients from the hyperthyroid group (11%). The most common complication was hypocalcemia. The choice of the treatments, rate of the complications, and the hospitalization periods, were not different between the groups. We conclude that hyperthyroidism does not cause any harm where the appropriate preoperative preparation and meticulous surgical tecniques were available.
