Tip 2 diyabetes mellituslu hastalardaki testosteron düzeylerinin kardiyovasküler risk faktörleri ile ilişkisi

Özet
Amaç: Çalışmamızda diyabetik hastalardaki testosteron düzeylerini diyabetik olmayan hastaların testosteron düzeyleri ile karşılaştırmak ve testosteron ile diyabet regülasyonu ve kardiyovasküler risk faktörleri arasındaki ilişkiyi araştırmak amaçlanmıştır. Gereç ve Yöntem: Çalışmamıza 65'i diyabetik 40'ı sağlıklı 105 kişi alındı. Vakalarımızın tamamı erkekti. Hastalar diabetes mellitus (DM) nedeniyle anti-diyabetik ilaç kullanmaktaydı. Başvuran diyabetik hastaların diyabet süreleri, cins ve yaş kayıtları yapıldı. Lipid profilleri incelendi. Testosteron, HbA1c, kan şekeri, ürik asit, kreatinin düzeylerinin tayini için kanları alındı. Bulgular: Diabetes mellitusu olan hastaların testosteron değeri ortalamaları 3.56±1.24 ng/dL iken DM olmayan grubun ortalamalarını 4.55±2.02 ng/dL olarak bulduk. Iki grup arasında istatistiksel olarak anlamlı fark vardı ve p=0.003 tespit edildi. Testosteron ile yaş, HbA1c, glikoz, ürik asit, total kolesterol, LDL kolesterol, kreatinin, DM süresi (yıl) arasında istatistiksel olarak anlamlı korelasyon yoktu. Testosteron ile trigliserid arasında negatif yönde (r=-0.204 p=0.043), HDL ile ise pozitif yönde (r=0.353 p=0.0001) anlamlı korelasyon gözlendi. Sonuç: Çalışmamızda testosteron düzeylerini diyabetik erkek hastalarda diyabetik olmayanlara göre daha düşük bulduk. Diyabet regülasyonu ile testosteron düzeyleri arasında korelasyon yoktu. Kardiyovasküler risk faktörlerinden trigliserit yüksekliği ve HDL düşüklüğü ile testosteron düzeyleri arasında anlamlı korelasyon olduğu tespit ettik. Çok yönlü olarak değerlendirilebilecek araştırmamızda testosteron düzeyleri düşük olanlarda koroner arter hastalığı riskinin arttığı, kolesterol düzeyleri yüksek olanlarda testosteron düzeylerinin düşük olduğu ve düşük testosteron düzeylerinin diyabetik hastalardaki infertilitenin bir basamağı olduğu sonuçlarına vardık.
Abstract
Objective: The goal of this study was to examine the relationship between testosterone levels of men with diabetes and diabetes regulation and cardiovascular risk factors. Material and Methods: 105 men were enrolled in this study. 65 of them were patients with DM who were receiving treatment of antidiabetic drugs. Their ages, diabet periods and diabet types were recorded. Their lipid profiles were evaluated. Blood samples were taken for testosterone, HbA1c, glucose, uric acid, creatinin analysis. Results: While the mean values of testosterone levels in diabetic patients were 3.56±1.14 ng/dL, it was 4.55±2.02 ng/dL in non diabetic patients. Statistically significant difference is found between those two groups (p=0.003). There were no significant correlation between testosteron levels with the age, HbA1c, glucose, uric acid, total cholesterol, LDL cholesterol, creatinin levels or DM periods. A negative corelation is found between testosterone levels and trigliserid levels (r =0.204, p = 0.043). Testosterone levels are positively correlated with HDL levels (r = 0.353, p= 0.001). Conclusion: Results show that men with DM have lower testosteron levels compared to men without a history of diabetes. There were no correlation between diabetes regulation and testosterone levels. A significant correlation is found between testosterone levels and high trigliserid levels and low HDL levels that are among cardiovascular risk factors. It can be assumed from those results that patients who have low testosterone levels may have a high potency of coronary artery disease risk. High cholesterol levels and low testosterone levels may be a risk for infertility. Our study points out that diabetic patiens who have low testosterone levels also have dyslipidemia which is consistent with the results of other studies.
