Özet

Amaç: Tiroid nodülleri en sık görülen tiroid hastalığıdır. Total tiroidektomi günümüzde benign bilateral nodüler tiroid hastalığının tedavisinde dünya genelinde kabul edilen cerrahi girişimdir. Hipokalsemi total tiroidektominin en sık rastlanan komplikasyonudur. Bu çalışmamızda, benign hastalık nedeniyle tiroidektomi uygulanan hastalarda postoperatif erken dönem hipokalsemi gelişmesinde etkili olabilecek faktörleri incelemeyi amaçladık. Gereç ve Yöntem: İzmir Atatürk Eğitim ve Araştırma Hastanesi 1. Cerrahi Kliniği’nde Şubat 2007 ve Ocak 2009 tarihleri arasında opere edilen malignite dışı ardışık 100 guatr olgusu retrospektif olarak değerlendirilerek, hipokalsemi gelişimine etki eden faktörler araştırıldı. Hastalar, postoperatif dönemde, hipokalsemi gelişen ve gelişmeyen olmak üzere iki gruba ayrıldı. Hipokalsemi gelişimi üzerine etki eden faktörleri saptamak amacıyla hastaların yaş, cinsiyet, ameliyat tipi, nodül çapı ve ameliyat öncesi tiroid fonksiyonları karşılaştırıldı. Bulgular: Çalışmaya dahil edilen 100 olgunun 81’i kadın (%81), 19’u erkek (%19) olup yaş ortalaması 46.4±11 (19-71) olarak tespit edildi. Yetmiş bir olguya total tiroidektomi (TT), 22 olguya tek taraflı lobektomi, 6 olguya tek taraflı lobektomi ve diğer tarafa subtotal tiroidektomi ve bir olguya da bilateral subtotal tiroidektomi uygulandı. Hipokalsemi gelişen ve gelişmeyen hastalar karşılaştırıldığında hipokalsemi gelişmesinde yaş, ameliyat tipi ve preoperatif nodül çapının etkili faktörler olduğu saptandı. Sonuç: Postoperatif geçici hipokalsemi benign hastalık nedeniyle uygulanan tiroidektomilerden sonra karşılaşılan nadir olmayan bir komplikasyondur. Total tiroidektomi hipokalsemi oranlarını artırmakla beraber cerrahi tekniklerin gelişmesi ve postoperatif erken dönemde önlem alınması, bu komplikasyonun ciddi sonuçlar oluşturmasını engelleyebilir. Bunun yanı sıra ameliyat tipi dışında hastanın yaşı, preoperatif nodül çapı hipokalsemi gelişiminde etkili faktörler olarak görünmekle birlikte bu konuda daha çok hastayı içeren prospektif çalışmalar yapılması gerektiği kanaatindeyiz.

Abstract

Aim: Thyroid nodule is the most common form of thyroid disease. Currently, total thyroidectomy is the worldwide accepted surgical procedure in the treatment of benign bilateral nodular thyroid disease. Hypocalcaemia is the most common complication of this procedure. In our study, factors that may influence the development of postoperative, early-stage hypocalcaemia in patients who underwent thyroidectomy due to benign disease were investigated. Materials and Methods: In order to determine risk factors in the development of hypocalcaemia, we retrospectively evaluated 100 consecutive patients with benign thyroid disease who were operated in 1. Surgery Clinic, Izmir Atatürk Training and Research Hospital during February 2007 and January 2009. Patients were classified into two groups according to the development or nondevelopment of hypocalcaemia in the postoperative period. In order to determine the factors affecting the development of hypocalcaemia, age, gender, type of surgery, nodule size and thyroid function before surgery were compared among patients with and without hypocalcaemia. Results: Among the 100 patients included in the study, 81 were women (81%) and 19 men (19%) with a mean age of 46.46 (± 11.97) (19-71). Total thyroidectomy performed in 71 of the cases, whereas 22 patients underwent unilateral lobectomy, six cases had unilateral lobectomy on one side and subtotal thyroidectomy on the other. Bilateral subtotal thyroidectomy was performed in one case. Our analysis determined age, type of surgery and preoperative nodal size as risk factors in the development of hypocalcaemia. Conclusion: Postoperative transient hypocalcaemia after thyroidectomy due to benign disease is not a rare complication. Although total thyroidectomy increases the rate of hypocalcaemia, serious complication can be avoided with the improvement of surgical techniques and preventive measures in postoperative early stage. Apart from the type of surgery, patient age, preoperative nodule diameters seem to be factors effecting the development of hypocalcaemia. Additional large prospective studies are needed to confirm our results.
