Tiroid ince iğne aspirasyon biyopsisi sonuçları ile cerrahi patoloji sonuçlarının karşılaştırılması

Özet

Amaç: Tiroid ince iğne aspirasyon biyopsisi (İİAB), benign tiroid nodüllerinin malign tiroid nodüllerinden ayrımında en güvenilir teşhis yöntemi olarak kabul edilmektedir. Bu çalışmada, tiroid İİAB ile tiroidektomi materyali sitopatoloji sonuçlarını karşılaştırarak, hastanemiz koşullarında tiroid lezyonlarının değerlendirilmesinde İİAB’nin etkinliğinin saptanmasını amaçladık. Gereç ve Yöntem: Tiroidektomi yapılan 505 hastanın 423’ü (%83,8) kadın, 82’si (%16,2) erkek, yaş ortalaması 46,6±12,2 (15-79) yıl idi. Ameliyat materyali patolojisi hastaların 421’inde (%83,4) benign, 84’ünde (%16,6) malign olarak bildirilmişti. İİAB sonrası tiroidektomi yapılan 353 hastanın sitoloji sonuçları ile ameliyat patoloji sonuçları karşılaştırıldı. Bulgular: İİAB sonuçları benign, şüpheli, malign ve yetersiz olarak sınıflandırıldı; buna göre 285’i (%80,7) benign, 48’i (%13.65).şüpheli, 15’i (%4,2) yetersiz ve 5’i (%1,4) malign olarak değerlendirildi. İİAB’si şüpheli hastaların karşılaştırma dışı bırakıldığı değerlendirmede İİAB’nin duyarlılığı %8, özgüllüğü %99 bulundu. Tiroid İİAB sonucu şüpheli rapor edilen 48 hastanın 11’inin ameliyat patolojisi malign rapor edildi. İİAB’si şüpheli hastaların malign kabul edildiği karşılaştırmada İİAB’nin duyarlılığı %25, özgüllüğü %86 olarak saptandı. Sonuç: Tiroid nodüllerinin ameliyat öncesi değerlendirilmesinde İİAB altın standart olarak bildirilmektedir. Oysa bu çalışmada elde edilen veriler, ameliyat gerekliliğinin belirlenmesi ve malignitenin doğrulanmasında İİAB’nin tek başına yeterli olduğu varsayımını desteklememektedir. Hastanemiz koşullarında tiroid İİAB’si, nedeni iyi bilinmemekle birlikte, malignite tanısında yeterli duyarlılıkta bir test değildir.

Abstract

Aim: Fine needle aspiration biopsy (FNAB) is considered to be the most trustworthy diagnostic method in differentiation of benign and malign thyroid nodules. By comparing thyroid FNAB cytology and thyroidectomy pathology results, we aimed to determine the utility of FNAB in the evaluation of thyroid lesions. Materials and Methods: Among 505 patients who underwent thyroidectomy, 423 were female (83,8%) and 82 (16,2%) were male. Median age was 46,6 (15-79). Pathology in patients who underwent thyroidectomy was reported as benign in 421 (83,4%) and malignant in the remaining 84 (16,6%) patients. The FNAB pathology results of 353 patients who were subjected to FNAB before thyroidectomy were compared. with the results obtained on the thyroidectomy samples. Results: FNAB results were categorised as benign, suspicious, malignant and insufficient for analysis. 285 (80,7%) of the samples were evaluated as benign, 48 (13,65%) as suspicious, 15 (4,2%) as insufficient for analysis and 5 (1,4%) as malignant. When the results were assessed after exclusion of patients with suspicion of malignancy, the sensitivity of FNAB was found as 8% and its specificity as 99%. Conclusion: The results indicate that in cases where thyroid FNAB results are reported as suspicious for malignancy, surgery should be recommended for definite diagnosis of the malignancy. Although FNAB is reported to be “the golden standard” in the pre-operative evaluation of thyroid nodules, our data do not support the utility of FNAB as the sole criterion in the decision for surgery. At least in our hospital conditions, thyroid FNAB is not a test with sufficient sensitivity in the diagnosis of malignancy.
