Özet

Sekonder hiperparatiroidi, uzun süre diyaliz tedavisi altındaki hastalarda gelişen kronik böbrek hastalığında aşırı parathormon artışı ve paratiroid hiperplazisi ile seyreden bir mineral kemik bozukluğudur. On altı yaşında olan kadın hastamız reflü nefropatisine bağlı son dönem böbrek yetmezliği tanısı ile 10 yıldır haftada üç gün hemodiyaliz tedavisi görmekteydi. Son dört yıldır giderek artan yaygın kemik ağrıları olmaya başlamış. Dizlerde genu valgum ve ayak bileklerinde varus deformitesi gelişmesi sonucu yürüyemez hale gelmiş. Paratiroid ultrasonografisi ve sintigrafisinde tiroid sol lob inferiorunda hiperplastik paratiroid bezi, kemik grafilerinde subperiostal kemik rezopsiyonunun artmış olduğu ve radyolusens güve yeniği görünümünün eşlik ettiği kemik deformiteleri saptandı. Medikal tedaviye dirençli sekonder hiperparatiroidi tanısı alan hastaya subtotal paratiroidektomi uygulandı. Subtotal paratiroidektomiden 6 ay sonra hastanın her iki dizine suprakondiler osteotomi ve wedge rezeksiyon yapılıp açık kondiler plakla osteotomi hattı tespit edildi. Ayak bilekleri deformitesi için, fizik tedavi sonrası ortopedik bot kullandırılan hasta yardımsız yürümeye başladı. Sonuç olarak, özellikle çocuklarda kontrolsüz sekonder hiperparatiroidi, sistemik komplikasyonların yanı sıra gelişen kemik deformiteleri ile hastaların yaşam kalitesini bozan bir dizi sakatlıklara yol açabilir.

Abstract

Secondary hyperparathyroidism is a mineral bone disorder with excessive parathormone secretion and parathyroid hyperplasia. This condition develops in patients with chronic kidney disease who receive long term dialysis. Secondary hyperparathyroidism can cause systemic as well as skeletal complications. We present a 16 years old female patient who had been receiving hemodialysis three times a week for 10 years. The signs and symptoms of the patient became increasingly disturbing in the last fours years, with bone pain, inability to walk due to the genu valgum at the knees and varus deformities at the ankles. Parathyroid ultrasonography and scintigraphy revealed a hyperplastic parathyroid gland inferior to the thyroid left lobe.. Bone deformities with increased radioluscence and moth hole appearance were detected in bone x-rays.The patient was diagnosed as a severe case of drug resistant secondary hyperparathyroidism. A subtotal parathyroidectomy was performed and 6 months later, the patient underwent wedge resection of both knees and supracondylar osteotomy. The osteotomy line was fixed by open condylar plate. With the additional help of physiotherapy of the ankles and orthopedic boots the, patien started to walk without help. In conclusion, especially in children, uncontrollable secondary hyperparathyroidism is a condition which affects quality of life by causing systemic complications and bone deformities. Although the disease has a progressive course, it is possible to treat patients even at the late phase.
