Tiroid cerrahisinde nöromönitorizasyonun rekürren larengeal sinir hasarı oranına etkisi

Özet
Amaç: Prospektif randomize çalışmamızda tiroidektomi uygulanan hastalarda intraoperatif sinir monitörü kullanımının rekürren larengeal sinirin (RLS) identifikasyonu, RLS hasarı, operasyon ve sinire ulaşma süreleri açılarından etkili olup olmadığını incelemeyi amaçladık. Gereç ve Yöntem: Çalışmaya, Mersin Üniversitesi Genel Cerrahi polikliniğine başvuran ve tiroidektomi planlanan 105 ardışık hasta alındı. Hastalar, nöromonitörizasyon ile RLS diseksiyonu yapılan (grup 1; 53 hasta) ve vizüel RLS diseksiyonu yapılan (grup 2; 52 hasta) olmak üzere randomize olarak iki gruba ayrıldı. Hastaların yaş, cinsiyet, preoperatif ve postoperatif patoloji sonuçları, yapılan ameliyatlar, preoperatif ve postoperatif vokal kord muayeneleri, sinir bulma öngörüleri, sinir bulma oranları, sinire ulaşma süresi, operasyon süresi, geçici ve kalıcı RLS hasarı ve komplikasyonlar istatistiksel olarak incelendi. Sonuçlar: Her iki grup arasında yaş, cinsiyet, preoperatif ve postoperatif patoloji sonuçları, yapılan ameliyatlar, sinir bulma öngörüleri, sinir bulma oranları, geçici/kalıcı RLS hasarı ve komplikasyonlar açısından fark saptanmadı (p>0.05, tüm parametreler için). Hastalarda istatistiksel olarak fark olmasa da sayısal ve yüzde olarak RLS’nin tanımlanması ve ulaşılmasında farklılıklar olduğu görüldü. Her iki grupta sinir bulunamayan hastalarda anlamlı olarak daha fazla sayıda geçici ve kalıcı sinir hasarı tespit edildi (p=0.001). Nöromonitörizasyonun hem sinire ulaşma süresinde hem de operasyon süresinde belirgin azalma sağladığı saptandı (p=0.001, p=0.001 sırasıyla). Yorum: Çalışmamızda elde edilen sonuçlar, nöromonitörizasyonun RLS’nin intraoperatif saptanmasını hızlandıran bir yöntem olduğu yönündedir. Bu yöntemin, postoperatif RLS hasarı üzerine olumlu, ancak istatistiksel olarak anlamlı olmayan bir etkisi mevcuttur.
Abstract
Aim: In this prospective randomized study, we aimed to analyze the benefits of using intraoperative nerve monitoring (IONM) in reducing injury to the recurrent laryngeal nerve (RLN), in shortening identification time of RLN, and in shortening the duration of a thyroidectomy operation. Materials and methods: 105 consecutive patients who were admitted for thyroidectomy to the Department of General Surgery of the Mersin University Medical Faculty were included in this study. The patients were randomly divided into two groups, as those who would be subjected to neuromonitored dissection of RLN (group 1; 53 patients) and those who would be subjected to visual dissection of RLN (group 2; 52 patients). In both groups of patients, age, gender, pre-operative and postoperative pathology results, previously performed operations, pre-operative and post-operative vocal cord examinations, predictions for the identification of RLS, the range of identification of the nerve, the time for reaching the nerve, duration of the operation, permanent or temporary injury of RLN and other complications were analyzed statistically. Results: There were no significant differences between the two groups in terms of age, gender, pre-operative and post-operative pathology results, pre-operative and post-operative vocal cord examinations, performed operations, predictions for the identification of RLS, the range of identification of the nerve, permanent or temporary injury of RLN and other complications (p>0.05, for all parameters). Although there was no statistically significant differences between the two groups in terms of identification of RLN, some minor differences in the percentages (96% vs 91%) were noted. In both groups significant RLN injury occurred in patients in whom RLN could not be identified (p=0.001). When the two groups were compared with regard to operation time and time needed for RLN identification, neuromonitorisation was found to significantly decrease both time spent for nerve identification (12.9 min vs 19.6 min, p=0.001) and total duration of the operation (99.5 min vs 144 min, p=0.001). Conclusion: The present study showed that neuromonitorisation of RLN is a method which shortens the time for reaching the nerve and consequently the total duration of thyroidectomy. It can also be considered a method which probably reduces the risk of post-operative RLN injury, although the results of this study on this issue were not statistically significant.
