Central neck dissection in the management of papillary thyroid cancer?

Özet

Tiroid kanserinin cerrahi tedavisinde asıl amaç, tiroid glandı içerisindeki ve etrafındaki makroskopik tüm hastalığı ve drene olan lenf nodlarını çıkarmaktır. Medüller tiroid kanseri tedavisinde cerrahi ve boyun diseksiyonu ile ilgili algoritmalar kesinleşmişken, tiroid papiller kanserinde boyun diseksiyonu konusu tartışmalıdır. Eski dönemde hemitiroidektomi mi yoksa total tiroidektomi mi tartışması yerini, santral boyun diseksiyonunun gerekli olup olmadığına bırakmıştır. Son zamanlarda en sık sorulan sorular; boyun diseksiyonunun endikasyonları, tipi (unilateral/bilateral veya terapötik/profilaktik), sürvi ve rekürrens üzerine etkisi ile ameliyat sonrası morbidite ve hasta yaşam kalitesi üzerine etkileridir. Bu çalışmanın amacı, papiller tiroid kanserinde yapılmış santral boyun diseksiyonu ile ilgili son çalışmaları değerlendirmek ve okuyuculara bu sorularla ilgili cevaplar verebilmektir.

Abstract

The main goal of surgery due to thyroid cancer is to remove all macroscopic disease in the gland, as well as draining lymph nodes and adjacent structures. As guidelines about extent of surgery and neck dissection in MTC patients are clear, all these are a matter of questions in PTC patients.The age-old debate about hemithyroidectomy versus total thyroidectomy for the treatment of PTC has now been replaced by the modern debate about the appropriateness of central neck dissection (CND).The main issues that have been recently raised are the following one: indications for neck dissection, type of the dissection /prophylactic or therapeutic, unilateral or bilateral/, impact of CND on survival and recurrence rate, as well as postoperative morbidity and quality of life. The aim of this article is to review the recent studies about CND in PTC patients in order to clarify some issues and to offer the readers main disputable points of this complex debate.
