Özet

Amaç: Gelişimsel kalça displazisinde (GKD) Pemberton perikapsüler osteotomisinin (PPO) erken dönem sonuçları değerlendirildi. Hastalar ve yöntemler: Gelişimsel kalça displazisi nedeniyle 15 hastanın (14 kız, 1 erkek; ort. yaş 2.5; dağılım 1.5-3 yaş) 20 kalçasına Pemberton perikapsüler osteotomisi uygulandı. Beş hastada iki taraflı tutulum vardı. Tönnis sınıflamasına göre 16 kalçada (%80) derece IV, dört kalçada (%20) derece III çıkık vardı. Hiçbir hastaya ameliyat öncesinde iskelet veya cilt traksiyonu uygulanmadı. Tüm kalçalarda osteotomi aralığına trikortikal trapezoid iliyak greft konularak K-teli ile tespit edildi. Yüksek kalça çıkıklı yedi hastada kısaltma ve derotasyon uygulandı. Hastaların ameliyat öncesi ve sonrası pelvis grafilerinden asetabuler indeks ölçüldü. Hastalar klinik olarak McKay ölçütlerine, femur başı avasküler nekrozu açısından Kalamchi-MacEwen ölçütlerine göre değerlendirildi. Ortalama takip süresi 3.6 yıl (dağılım 1-6 yıl) idi. Bulgular: Klinik olarak 10 kalçada (%50) mükemmel, sekiz kalçada (%40) iyi, iki kalçada (%10) orta sonuç alındı. Ameliyat öncesinde ortalama 40° olan asetabuler indeks, son takipte 18° ölçüldü. Kalamchi-MacEwen sınıflamasına göre altı kalçada (%30) tip I, üç kalçada (%15) tip II avasküler nekroz izlendi. Hiçbir olguda yineleyen subluksasyon veya çıkık gelişmedi, displazi saptanmadı. Sonuç: Pemberton perikapsüler osteotomisinin GKD tedavisinde kısa dönem sonuçları güvenli ve başarılı bulundu.

Abstract

Objectives: We evaluated the short-term results o Pemberton pericapsular osteotomy for the treatment o developmental dysplasia of the hip (DDH). Patients and methods: Fifteen patients (14 girls,boy; mean age 2.5 years; range 1.5 to 3 years) with DD underwent Pemberton pericapsular osteotomy in 20 hip Five patients had bilateral involvement. According to th Tönnis classification, 16 hips (80%) were rated as grad IV, and four hips (20%) were rated as grade III. None o the patients underwent skeletal or skin traction preop eratively. A tricortical trapezoidal iliac graft was place in the osteotomy space and was fixed with a K-wir Shortening and derotation were performed in seven of th high-riding hips. The acetabular index was measured o pre- and postoperative pelvic radiographs. Clinical evalu ations were made according to the McKay criteria, an avascular necrosis of the femur head was assessed usin the Kalamchi-MacEwen criteria. The mean follow-u period was 3.6 years (range 1 to 6 years). Results: Clinical results were excellent in 10 hips (50% good in eight hips (40%), and moderate in two hips (10% The mean acetabular index decreased from 40° to 18 postoperatively. According to the Kalamchi-MacEwe criteria, six hips (30%) developed type I, three hips (15% developed type II avascular necrosis. Postoperative dislo cation, subluxation, or dysplasia were not detected in an of the patients. Conclusion: Pemberton pericapsular osteotomy wa found to be safe and successful in the short-term follow up of patients with DDH.
