Koruyucu parenteral K vitamini uygulamasının sağlıklı yenidoğan bebeklerdeki hiperkoagulabiliteye etkisi: Preliminer çalışma

Özet
Yenidoğan bebeklerde hemostaz dengesinin oldukça hassas olduğu ve kanama riski yanında tromboz riskinin de önemli bir problem olduğu bilinmektedir. Bu çalışmanın amacı sağlıklı yenidoğan matür bebeklere parenteral uygulanan vit K profilaksisi ile in vitro olarak hiperkoagulabilitenin nasıl etkilendiğini araştırmaktı. Hiperkoagulabilite kriterleri olarak Trombin-antitrombin kompleks (TAT) ve Protrombin fragment 1,2 (PF) kullanıldı. K vit etkisi PIVKA-II ölçümü ile denetlendi. Ayrıca APC-Rezistansı (APC-R) yanında Protein Z ölçümleri K vit uygulaması ve sonrasında yapılarak karşılaştırıldı. Tüm bebekler matür olup 4'ü kız, 57 erkek olmak üzere 9 bebek çalışmaya alındı. K vit sonrası PIVKA-II düzeylerinin anlamlı olarak azalmasına karşın (34.6±74.1/K vitamini sonrası 27.1±68.1 ng/ml)(p=0.04), APC-R (101±8.2/99.4±5.1%), TAT (25.0±28.9/32.2±34.1 mcg/L), PF (2.0±1.1/2.9±3.2 nmol/L) ve Protein Z (23.5±10.2 / 21.3±8.4 ng/ml) düzeyleri arasında K vit öncesi ve sonrası düzeyler açısından anlamlı fark bulunmadı. Sonuç olarak K vit uygulaması ile sağlıklı yenidoğanlarda tromboza eğilim artışı saptanmadı. Yenidoğanın hemorajik hastalığına karşı K vit profilaksisi ihmal edilmemeli ve tek doz intramuskuler olarak her doğum sonrası uygulanmalıdır. Söz konusu uygulama ile tromboz riskinin artmadığı in vitro olarak gösterilmiştir.
Abstract
The neonates has an unique balance of hemostasis and they may prone either bleedings or thrombotic attacks in the neonatal period. The purpose of this study was to investigate the effects of vitamin K in the healthy neonates in respect of hypercoagulability. For hypercoagulability markers, Trombin-antitrombin complex (TAT) and Protrombin fragment 1,2 (PF) were used. PIVKA-II ELISA test was used for vitamin K effect on acarboxylated protrombin. All study patients were healthy mature neonates and four of them were female. Totally nine babies were enrolled the study after parent consent. Laboratory tests were used before and after (mean 10 hours) K vit injection by citrated blood samples. PIVKA-II levels were found significantly decreased after K vit (34.6±74.1/after K vit 27.1±68.1 ng/ml)(p=0.04) whereas TAT (25.0±28.9/32.2±34.1 mcg/L), PF (2.0±1.1/2.9±3.2 nmol/L), Protein Z (23.5±10.2/21.3±8.4 ng/ml) and APC-Resistance ratios (101±8.2/99,4±5.1 %) were not different after vit K administration. As a conclusion, vit K injection was not found to increase the hypercoagulability status in healthy neonates. Vit K prophylactic administration is very useful and necessary procedure for every neonates. No trombotic effect was determined after K vit injection, in vitro.
