Özet

Moniliazis, kronik diare, amoebiazis ve yineleyen akciğer infeksiyonları olan 4 aylık erkek olgu SCID (T-, B-) tanısı aldı. Lenfosit panelinde total T ve B hücresi (CD3 ve CD19) ve HLA-DR değerleri %0, NK hücresi %78 bulundu. Burst testi normal, pokeweed mitojen ve conconavalin A'ya mitojenik yanıt ise negatif idi. Adenozin deaminaz eksikliği yoktu. Transplantasyon (TX) öncesi ve sonrasında CMV antijenemisi saptanmadı. Olguya, 7 aylık iken, aile bireyleri içinde karışık lenfosit kültürü reaktif olmayan erkek kardeşinden (11 yaş), ailenin de tercihi ile periferik kan kök hücre nakli uygulandı. Herhangi bir hazırlama rejimi verilmedi. Donöre G-CSF 5µg/kg 5 gün süre ile verildi. Üründe MNH 4x108/kg, CD34+ hücre 4.93x106/kg idi. Post TX +14. günden sonra kan CD3, CD4 ve CD8 değerleri normale yükseldi, ateşi düştü kr. diaresi ve genel durumu hızla düzeldi ve belirgin kilo artışı oldu. Öyküsünde 2 aylık iken BCG aşısı yapıldığı belirtilen, ancak BCG skarı gözlenmeyen hastada, +14-21. günlerde BCG püstülüzasyonu ve sol aksiller BCG LAP gelişti. TX öncesi ve sonrası Tb ve CMV infeksiyonu saptanmadı. İzlemde, ciltte 5 gün süren akut graft versus host hastalığı (AGVHD) (Gr-1) gelişti. +51. günde BCG LAP total eksize edildi ve dokuda M. bow's suşu üretildi, izlemde, KCFT bozukluğu nedeni ile siklosporin A ve antiTb 45 gün verilebildi. PPD testi +150. günde 10 mm bulundu. Şu anda +34. ayda bulunan olgunun lenfosit panelinde CD3 (%35-48), CD4 (%41-66) ve CD8 (% 19-29) ve CD19 %9 düzeylerinin stabil kaldığı HLA-DR ve CD25'in giderek azaldığı görüldü. IVIG uygulamasının kesilmesinden 8 hafta sonrasından itibaren serum IgG düzeylerinin yeterli düzeylerde olduğu dikkat çekti. Donör kaynaklı lenfositlerle gecikmiş BCG yanıtı ve BCG LAP ilginç bulundu.

Abstract

A 4 month-old male patient with moniliasis, chronic diarrhea, amoebiasis and recurrent lung infections was diagnosed as SCID (T-B-). The total T and B cells (CD3 and CD19) and HLA-DR values were found to be 0%, NK cells 78% in the lymphocyte panels. The burst test was normal while mitogenic responses to pokeweed mitogen and conconavalin A were negative. Adenosin deaminase deficiency was not present. CMV antigenemia was not found before or after the transplantation (TX). When he was 7 months old, a peripheric blood stem cell transplantation was performed from his brother, 11 years old, whose lymphocyte culture was not reactive. No preparatory regimen was given. rhuG-CSF (5 µg/kg) was given to the donor for 5 days. In the product, MNC and CD34+ cell counts were 4x108/kg and 4.93x106/kg, respectively. Beginning on day 14 after transplantation, the blood levels of CD3, CD4 and CDS reached to normal. His fever dropped, chronic diarrhea and his general condition improved dramatically. A significant weight increase was also noted. The patient having a history of BCG vaccination at 2 months, but with no visible scarring, developed BCG pustulisation on day 14, and left axillary BCG LAP on day 21. Tb and CMV infections were not found before or after TX. In the follow up, AGVHD (Gr-1) developed on the skin lasting for 5 days. On day 51, a total excision of BCG LAP was performed and M. bow's was cultured in the tissue. During this period cyclosporin A and anti-Tb therapy were given for just 45 days because of the elevated liver function tests. On day 820 post-tx, CD3 (35-48%), CD4 (41-66%), CDS (19-29%) and CD19 (%9) rates remained stable, HLA-DR and CD25 rates were gradually decreased. After having stopped IVIG, sufficient levels of IgG were found. It was interesting to observe a delayed BCG response and BCG LAP development due to donor lymphocytes.
