Nötropenik sepsisli prematürelerde rekombinant insan granülosit-makrofaj-koloni stimülan faktör (rhGM-CSF) tedavisi prognozu düzeltilebilir

Özet
Sepsis yenidoğan döneminde başlıca mortalite ve morbidite nedenidir. Yenidoğan, nötrofil depo havuzunun azlığı, kemik iliği granülosit prekürsörlerinin yetersiz olması, nötrofil fonksiyonlarının bozuk olması nedeniyle sepsis gibi stres durumlarında hızla nötropeniye girer. rhGM-CSF'ün nötrofil sayısını arttırdığı ve fonksiyonlarını düzelttiği gösterilmiştir. Biz bu çalışmada, rhGM-CSF'ün nötropenik sepsisli prematürelerde nötrofil sayısı ve prognoz üzerine etkisini araştırmayı amaçladık. Çalışmaya alınan 30 nötropenik sepsisli prematüre bebekten 15'ine rhGM-CSF (10 µgr/kg/gün, 5 gün), antibiyotik ve destek tedavisi, diğer 15 bebeğe sadece antibiyotik ve destek tedavisi verildi. Tüm olgularda absolü nötrofil sayısı (ANS), band/PNL, lenfosit, eozinofil, monosit ve trombosit sayıları 0., 24., 72. saat ve 5. ve 10. günlerde sayıldı. Abstract
Sepsis is the major cause of the mortality and morbidity in neonates. Preterm neonates have both quantitative and qualitative neutrophil defects with decreased bone marrow neutrophil storage pool (NSP) reserves, an inability to increase neutrophil production, and defective neutrophil functional activity. The study was aimed to the effect of treatment with recombinant human granulocyt-macrophage colony stimulating factor (rhGM-CSF) on neutrophil counts and on prognosis of preterm neonates with documented sepsis. Thirty septic neutropenic preterm infants were randomized for rh GM-CSF therapy 10 mcg/kg/d for 5 days. All infants got antibiotic therapy and supportive care. Absolute neutrophil count (ANC), immature to total neutrophil ratio (I/T), lymphocyte, eosinophil, monocyte and thrombocyte counts were measured in all septic neonates on days 0, 1, 3, 5 and 10.
