Nedeni bilinmeyen çocukluk çağı senkoplarının incelenmesinde tilt testinin ve edrofonyum ile indüksiyonun yerini araştırmak amacıyla yapılan çalışmamızda senkop nedeni ile başvuran 23 olgu ve kontrol grubunu oluşturan senkop öyküsü olmayan ve yaş ve cinsiyet açısından fark olmayan 10 sağlıklı çocuk değerlendirildi. Altmış derecede 25 dakika süren bazal tilt testi sonrasında 0.2 mg/kg (maksimum 10 mg) edrofonyum intravenöz olarak uygulandı. Yirmi üç olgunun 14'ünde (%60.8) pozitif yanıt görüldü. Bazal tilt testi sırasında dört olguda, edrofonyum uygulaması ile 9 olguda senkop gözlendi. Kontrol grubunu oluşturan 10 olgunun birinde bazal test sırasında pozitif yanıt görüldü; edrofonyum uygulaması ile dokuz olgunun hiçbirinde senkop görülmedi. Böylece çocuklarda edrofonyum indüksiyonlu tilt testinin duyarlılığı %60.8, özgüllüğü %90.0 olarak hesaplandı. Edrofonyuma bağlı önemli bir yan etki görülmedi. Edrofonyum indüksiyonlu tilt testinin nöral güdümlü refleksif senkop geçiren hastaları belirleme oranını önemli derecede arttırdığı, emniyetli ve zamandan kazanç sağlayan bir yöntem olduğu kanısına varıldı.

Abstract

Twenty-three consequtive patients with a history of syncope and 10 healthy children, without a history of syncope were enrolled in the study to investigate the place of tilt testing and edrophonium induction in the evaluation of unexplained syncope during childhood. Edrophonium was administered intravenously (0.2 mg/kg, maximum 10 mg) after 25 minutes of basal tilt testing at 60°. A positive response was observed in 14 of 23 (60.8 %) patients. Four patients experienced syncope during basal tilt testing and nine did so with edrophonium administration. Syncope was induced in one of 10 control subjects; and none of the remaining nine did so with edrophonium bolus. Hence, the sensivitity and the specificity of tilt testing with edrophonium induction in children was found to be 60.8 % and 90.0 %, respectively. There were no significant side effects from using edrophonium. It is concluded that tilt testing with edrophonium induction significantly increases the identification rate of patients with neurally -mediated reflexive syncope, and is a safe and a time - conserving method.
