Matür yenidoğanlarda sepsisde mikroorganizmaların dağılımı

Özet
Yenidoğan sepsisi neonatal mortalité ve morbiditenin en önemli nedenidir. Neonatal sepsise yol açan mikroorganizmalar periyodik olarak değişir. Çalışma bu nedenle planlandı. Ocak 1996-Aralık 1997 tarihleri arasında, izmir Dr. Behçet Uz Çocuk Hastanesi'ndeki 189 sepsisli matür yenidoğan retrospektif olarak değerlendirildi. Sepsis tanısı klinik muayene ve kan kültüründen izolasyonla konuldu. Ocak 1996-Aralık 1997 arasında, hastanede yatan 3932 matür yenidoğandan 189 (%4.8)'u sepsis tanısı aldı. Sepsis oranı 1996'da %6.3 iken 1997'de %3.4 bulundu. Sepsisli hastaların 65 (%34.4)'i kız, 124 (%65.6)'ü erkek idi. Hastalar ortalama 6.6±7.2 günlükken hastaneye yatırıldılar.Sepsis tanısı 7.2±7.9 günlükken konuldu. Kan kültürü ile tanınan 189 episoddan 68 (%36)'i erken başlangıçtı (doğumdan sonraki ilk 96 saat içinde) ve 121 (%64)'i geç başlangıçlı (doğumdan 96 saat sonra) sepsis olarak bulundu. Sepsisli hastaların 22 (%11.6)'sı nozokomiyal enfeksiyon olarak değerlendirildi. Hastaların 11 (%5.82)'i menenjitle birlikte sepsis olarak değerlendirildi. Neonatal sepsisli 189 hasta içinde yaygın olarak Stafilokokus aureus (%47.9), Kiebsiella (%31.9), E.Coli (%6.9) ve Stafilokokus koagülaz negatif (%5.9) izole edildi. Mortalité oranı %39.6 olarak bulundu. Neonatal sepsisin patojenlerine ait veriler değişmektedir. Bu nedenle, veriler periyodik olarak yeniden gözden geçirilmeli ve ona göre antibiyotik kullanımı düzenlenmelidir.
Abstract
Neonatal sepsis is an important reason of neonatal mortality and morbidity. Causing microorganisms of neonatal sepsis are being changed periodically. This study planned for this manaer. Between January 1996 and December 1997, 189 mature newborn with sepsis in izmir Dr. Behçet Uz Children's Hospital were evaluated retrospectively to this study. Sepsis was diagnosed with clinical examination and isolation in the blood culture. Between January 1996 and December 1997, 189 (4.8%) patients from 3932 hospitalized mature newborns were diagnosed sepsis. While the percentage of sepsis was finded 6.3%in 1996, that was finded 3.4%in 1997. There were 65 (34.4%) female, 124 (65.6%) male newborn in the patients with sepsis. Patients were hospitalized at age of 6.6+7.2 day. They were diagnosed sepsis at age of 7.2±7.9. Out of 189 episodes of blood culture proven sepsis 68 (36%) presented as early onset (within 96 hours of birth) and 121 (64%) as late onset sepsis (after 96 hours of birth). Twenty-two (11.6%) of patients with sepsis were evaluated as nosocomiai infection. Eleven (5.82%) of patients were evaluated as sepsis with menengitis. Among 189 cases of neonatal sepsis, the predominant isolates were Staphilococcus aureus (49.7%), Klebsiella (31.9%), E.coli (6.9%) and Coagulase negative Staphylococcus (5.9%). Mortality rate was founded as 39.6%. The data concerning pathogens of neonatal sepsis is being changed. So that, the data must be periodically reviewed and antibiotic policy revised accordingly.
