Özet

Yenidoğanlarda nozokomiyal sepsisin değerlendirilmesi amaçlandı. Bu çalışmada izmir Dr. Behçet Uz Çocuk Hastanesi Yenidoğan Bölümünde 1996-1997 tarihleri arasında yatan hastalar retrospektif olarak incelendi. Sepsis tanısı klinik olarak ve kan kültüründe üreme saptanması ile konuldu. Nozokomiyal sepsis tanıları Centers of for Diseases Control and Prevention (CDC) 1988 hastane enfeksiyonları tanım kriterlerine göre yapıldı. Bu süre içinde 4669 yenidoğan hastaneye yatırıldı. 392 hasta neonatal sepsis tanısı aldı. Nozokomiyal sepsis oranı 121 hasta ile %2.59 olarak bulundu. Nozokomiyal sepsisli hastaların %81.851 prematüre, %18.15'i matür yenidoğan idi. Prematürelerde, en sık izole edilen bakteriler Klebsiella (%42.0), Stafilokokus aureus (%22.0), Enterobacter (%9), Eco// (%8.0) idi. Matür yenidoğanlarda en sık izole edilen bakteriler Klebsiella (%68.2), Eco// (%13.6), Stafilokokus aureus (%9.1) idi. imipenem, meropenem ve amikasin en duyarlı antibiyotiklerdi. Penisilin, ampisilin, gentamisin ve piperasilin her iki grupta oldukça dirençli bulundu. Seftazidim, sefotaksım ve ampisilin-sulbaktam ise özellikle matür yenidoğan nozokomiyal sepsisinde dirençliydi. Nozokomiyal sepsisli hastalarda mortalité %17.3 olarak bulundu. Neonatal nazokomiyal sepsis ve antibiyotik duyarlılığına ait bilgiler periyodik olarak gözden geçirilmeli ve antibiyotik politikası buna göre düzenlenmelidir.

Abstract

We aimed to determine nosocomial sepsis in newborns. In this study, the newborns hospitalized in İzmir Dr. Behçet Uz Children's Hospital Neonotology Department were evaluated retrospectively between January 1996 and December 1997 nosocomial sepsis in newborns. Sepsis was diagnosed with clinical examination and isolation in the blood culture. The definition of nosocomial sepsis was done by using the criteria proposed by Centers for Disease Control and Prevention (CDC) in 1988. In this period, 4669 newborns were hospitalized. Three hundred nighty-two patients were diagnosed as sepsis. Nosocomial sepsis rate with 121 patients was finded 2.59%. Patients with nosocomial sepsis were distrubited premature (81.15%) and mature (18 85%) newborns. The most frequently isolated bacteriae were Klebsiella (42.0%), Staphylococcus aureus (22.0%), Eco// (8.0%) in premature newborns whereas the most frequently isolated bacteriae were Klebsiella (68.2%), E.coli (13.6%), Staphylococcus aureus (9.1%) in mature newborns. Penicilline, ampicillin, gentamicin and piperacilline were found as the most resistant antibiotics in both groups. Ceftazidim, cefotaxime and ampicillin-sulbactam were résistent to nosocomial sepsis in mature newborns. Mortality rate was found 17.3% due to septicemia. The data concerning pathogens of neonatal nosocomial sepsis and antibiotic sensivity must be periodically reviewed and antibiotic policy rmust be evised accordingly.
