Özet

Bu çalışmada Manisa Moris Şinasi Çocuk Hastanesinde izlenen ishalli çocuklardan izole edilen Shigella türü bakterilerin antibiyotik duyarlılıklarının araştırılması amaçlanmıştır. Bu amaçla akut enfeksiyöz diarelerde ampirik sağaltımda sık kullanılan ampisilin (Amp), trimethoprim-sülfametoksazol (SXT), kloramfenikol (C), kinolon (Ciprofloksasin: Cip) ve üçüncü jenerasyon sefalosporin (Cephtriaxon: CRO) test edilmiştir. Antibiyotik duyarlılıkları. disk difüzyon yöntemiyle araştırılmıştır. En sık izole edilen tür Shigella flexneri iken (%59.4), bunu Shigella sonne (%35.6), Shigella boydu (%3) ve Shigella dysenteria (% 2) izlemiştir. Tüm suşlar Cip ve CRO'a duyarlı bulunmuştur, izole edilen toplam 101 Shigella susunun Amp, SXT ve C'e direnç oranları ise sırasıyla %98, %60 ve %54 olarak saptanmıştır. Saptanan bu sonuçlar Shigella gastroenteritlerinde sağaltım gerektiğinde antibiyotik duyarlılığına bakılmasının gerekli olduğunu; ampisilinin bu amaçla kullanılmamasının ve SXT ile C için ise antibiyogram sonuçlarının beklenmesinin uygun olduğunu düşündürmektedir.

Abstract

In this study we aimed to determine the susceptibility of Shigella strains commonly isolated children having infectious diarrheae in Morris Schinassi Pédiatrie Hospital in Manisa to ampicillin (Amp), trimethoprim-sülfametoksazol (SXT), chloramphenicol (C), quinolones (Ciprofloksasin: dp) and third generation cephalosporines (Cephtriaxon: CRO) which are antibiotics used in empirical treatment of acute infectious diarrhea. Antimicrobial susceptibility was performed using disk diffusion method. S. flexneri (59.4%) was the most frequently isolated pathogen followed by S. sonnei (35.6%), S. boydii (3%) and S. dysenteria (2%). All strains were found susceptible to dp and CRO. The resistance percentages of totally 101 Shigella strains were found as %98, 60%, and 54% to Amp, SXT and C, respectively. From these results, it can be concluded that SXT and CRO are the drugs of choice in empirical therapy of Shigella gastroenteritis in children. In therapy of infectious diarrheae of known etiology, results of antimicrobial susceptibility must be taken into consideration for addresing the effective antimicrobial agent.
