Özet

Herediter sferositoz; eritrosit hücre membran iskelet proteinlerinin anomalisi ile karakterize kalıtsal hemolitik anemilerin sık görülen bir nedenidir. Klinik bulguları oldukça heterojendir. Asemptomatik olgular olabildiği gibi kompanse kronik hemoliz ve transfüzyon gerektiren ağır hemolitik anemi de görülebilmektedir. Bazı çocuklar asemptomatik kalırken diğerlerinde; ağır hemolitik anemiyle birlikte sarılık, splenomegali ve safra kesesinde taş oluşumu vardır. Labaratuvar bulgularında; hemolizin şiddetine paralel olarak değişen anemi, hiperbilurubinemi ve retikülositoz görülür. Ayrıca ortalama eritrosit hemoglobin konsantrasyonu ve osmotik fragilité sıklıkla artmış olarak saptanmaktadır. Tedavide ona ve ağır şiddette anemi ile seyreden olgularda splenektomi önerilmektedir. Çalışmamızda Dr. Behçet Uz Çocuk Hastalıkları ve Cerrrahisi Eğitim ve Araştırma Hastanesi Hematoloji bölümünde izlenen toplam 40 herediter sferositoz olgusunun klinik, laboratuvar özellikleri ve tedavi yaklaşımları sunulmuştur. Olguların 18'ine total splenektomi yapılmış ve cerrahi girişim sonrası izlemlerinde infeksiyon ve diğer komplikasyonların gelişme riski değerlendirilmiştir.

Abstract

Hereditary spherocytosis (HS) is a common inherited hemolytic anemia involving cell-membrane alterations. It's clinical expression is heterogeneous, ranging from clinically silent forms with well-compensated chronic hemolysis to severe transfusion-dependent anemia. Some children remain asymtomatic but others may have severe hemolytic anemia, splenomegaly, jaundice and gallstones. Laboratory findings are evidence for hemolysis includes reticulocytosis, hyperbilurubinemia. The mean corpuscular hemoglobin concentration and osmotic fragility tests are often increased. Splenectomy is thus the treatment of choice for moderate-to-severe forms of the disease. Increased awareness that splenectomized patients face a lifelong risk of overwhelming life-threatening infections has dampened enthusiasm for the routine use of total Splenectomy for the management of HS. We evaluated the clinical and laboratory features of 40 patients with HS followed by Dr.Behçet Uz Children Hospital Hematology Department. Of whom, 18 patients underwent total splenectomy and were monitored for infectious and other complications.
