Özet
Pulmoner alveolar proteinozis (PAP), surfaktan lipid ve proteinlerin alveoller ve distal hava yolunda birikimi ile karakterize etiyolojisi bilinmeyen nadir bir interstisyel akciğer hastalığıdır. İlk kez 1958'de tanımlanmıştır. Hastalığın primer formunun patogenezi tam olarak bilinmemekle beraber, monosit-makrofaj sisteminin granülosit-makrofaj koloni stimule edici faktöre (GM-CSF) cevapsızlığının yattığı düşünülmektedir. 13 aylık erkek olgu nefes almada zorluk ve morarma yakınması ile acil servise yatırıldı. Hikayesinde, 6 aylıktan itibaren tekrarlayan öksürük ve morarma şikayetleri ile hastaneye yatışının olduğu öğrenilen olgunun yapılan laboratuvar ve radyolojik incelemelerinde, interstisyel akciğer hastalığı düşünüldü. Yapılan akciğer biyopsisinin histopatolojik incelemesi sonucu pulmoner alveolar proteinozis lehine gelmesi üzerine nadir görülmesi nedeni ile sunuldu.
Abstract
Pulmonary alveolar proteinosis (PAP) is a rare chronic interstitial lung disease of unknown etiology characterized by accumulation of surfactant lipids and proteins in the alveoli and distal airways. It was first described in 1958. Although the pathogenesis of primary or idiopathic PAP remains unknown, it has been proposed that a loss of responsiveness of the monocyte-macrophage lineage to granulocyte-macrophage colony stimulating factor (GM-CSF) is involved in PAP. Thirteen month old boy was admitted to the emergency department because of dyspnea and cyanosis. From the history, we learned that he was admitted to the hospital for several times because of cough and cyanosis. With the laboratory and the radiolodical features, tha disease was taught that "interstitial lung disease". The histopathological examination of the lung biopsy material was reported as "pulmonary alveolar proteinosis". This case was presented because of its rarety.
