Özet

Agiri-pakigiri kompleksi serebral korteksteki girusların tam veya tama yakın yokluğu ile karakterli bir nöronal migrasyon anomalisidir. Bu malformasyonlar yaygın olabileceği gibi lokalize de olabilmektedirler. Nöbetler agiri-pakigiri kompleksi olan olgularda en sık gözlenen semptomlardandır. Bu çalışmanın amacı agiri-pakigiri malformasyonu olan olgularda lezyonun yaygınlığı ile nöbet özelliklerinin karşılaştırılmasıdır. Çalışmaya 37 hasta alınmış, manyetik rezonans görüntüleme bulgularına göre yaygın giral malformasyon gösteren 18 olgu grup 1 ve lokalize malformasyon gösteren 19 olgu grup 2 olarak ayrılmıştır. Olguların 22'si erkek, 15'i kız; yaş ortalamaları 21.09±31.24 aydır. Olguların %75.7'sinin nöbet geçirdiği saptanmış olup bunların %64.3'ünde jeneralize, %35.7'sinde parsiyel nöbet gözlenmiştir. Grup 1'de jeneralize, grup 2'de ise parsiyel nöbet geçiren olguların sayısı anlamlı olarak yüksek bulunmuştur. Nöbet başlangıç yaşı açısından gruplar arasında anlamlı farklılık saptanmamıştır. Grup 1 'deki olguların nöbet sıklığı grup 2'deki olgulara göre anlamlı olarak yüksek bulunmuştur. Nöbet geçiren olguların %75'inde antiepileptik tedavi ile nöbetlerde %50 ve daha fazla azalma görülmüştür. EEG bulguları yönünden değerlendirildiğinde jeneralize ve multifokal anormallikler grup 1 olgularda, fokal anormallikler grup 2 olgularda istatistiksel anlamlı olarak yüksek bulunmuştur. Bu çalışma yaygın agiri-pakigiri kompleksi olan olguların jeneralize tipte ve daha sık nöbet geçirme eğiliminde olduklarını ve bu olguların epilepsi cerrahisi gibi diğer tedavi yöntemleri yönünden değerlendirilmelerinin gerekliliğini düşündürmüştür.

Abstract

The term agyria-pachygyria is applied to disorders of neuronal migration anomalies that result in total or subtotal absence of cortical sulci. These malformations may be diffuse or localized. Seizures are the most common symptoms. The aim of this study is to compare the features of seizures with the extent of the lesions in these patients. On the basis of the MRI findings, 37 patients were separated into two groups: group 1 included 18 children with generalized gyral malformation and group 2 included 19 children with localized gyral malformation. There were 22 males and 15 females with the mean age o(21.09±31.24 months. Seizure disorders were present in 75.7% of the patients. Of them, 64.3% had generalized seizures and the remaining had partial seizures. The ratio of generalized seizures in group 1 was significantly higher, where partial seizures were more common in group 2. The difference in the onset of seizure disorder between the groups was not significant. The patients in group 1 had seizures significantly more frequent than group 2 patients. Of the patients, 75% were good-responders to antiepileptic treatment. Diffuse EEG abnormalities were significantly more common in group 1 and so do the localized abnormalities in group 2. In conclusion, this study showed that patients with diffuse gyral anomalies experienced often generalized seizures and the seizure frequency was much more higher; and other treatment options such as epilepsy surgery had to be considered in these patients.
