Özet

Bu çalışmada; zehirlenme nedeni ile başvuran olguların, epidemiyolojik ve klinik özelliklerinin prospektif oıarak değerlendirilmesi amaçlandı. 1.03.2003-31.08.2003 tarihleri arasmda Dr. Behçet Uz Çocuk Hastalıkları ve Cerrahisi Eğitim ve Araştırma Hastanesi Acil Servisine başvuran karbonmonoksit ve besin zehirlenmeleri dışındaki 129 zehirlenme olgusu yaş, cinsiyet, zehirlenme yeri, etkenin alınış yolu, şekli, özelliği, hastaneye başvuru süresi, geliş belirti ve bulguları, uygulanan tedavi yöntemleri, hastanede kalış süreleri ve sonuçlar yönünden praspektif olarak değerlendirildi. Zehirlenme olgularının yatan tüm hastalara oranı %12.7 olup hastaların yaş dağılımı 7ay-14 yıl (ortalama 4.1 yaş) arasında değişmekte idi. Olguların %60'ı başvuru sırasında asemptomatik iken, semptomatik olanlarda en sık fizik muayene bulgusu %24 oranı ile uykuya meyil olarak belirlendi. Zehirlenme etkeni %72 oranında farmakolojik bir ajandı ve santral sinir sistemi ilaçları %36 oranı ile ilk sırada yer aldı. Olguların %26'sı tedavi uygulanmaksızın izlenirken, en sık uygulanan tedavi gastrik lavaj ve aktif kömür uygulaması idi. Çocukluk yaş grubunun önlenebilir mortalite ve morbidite nedenleri arasında yer alan zehirlenmelerin azaltılabilmesi için daha fazla koruyucu önlem alınması gereklidir.

Abstract

In this study, we aimed to evaluate the epidemiological and the clinical characteristics of the children who attended to our hospital because of poisoning. We evaluated a hundred and twenty nine patients who attended to the Emergency Department of Dr. Behçet Uz Children's Hospital, because of poisoning except the food poisoning and carbon-monoxide exposure, between March 2003 and August 2003 prospectively. We examined the age and sex, the place of poisoning, the way of taking, the form and the characteristics of the toxic agent, the time of referral to the hospital, the symptoms and signs at the referral, the treatment methods, hospitalization period and outcome. According to our results the ratio of poisoning was 12.7%. The age of the cases was between 7 months and 14 years (median age 4.1 years) with a peak incidence at the age of 1-5 years. 60% of the cases were asymptomatic at the referral and the. most frequent sign among the symptomatic cases was tendency to sleep with the ratio of 24%. The pharmaceutical agents were taken most commonly and the central nervous system drugs (36%) took the first place in these agents. The most frequent therapy was gastric decontamination and activated charcoal and 26% of the cases were observed without any treatment, We need to take more preventions to decrease the incidence of childhood poisoning which takes an important place in the childhood mortality and morbidity.
