Özet

Pankreasın psödopapiller tümörü seyrek görülen kendine özgü klinikopatolojik bir antitedir. Lokal infiltrasyon, uzak metastaz ve rekürrens çok nadir olan tümör genelde adölesan kızlarda ve genç kadınlarda büyüyen abdominal kitle ve buna bağlı semptomlar ile tanınır. Ondört yaşındaki kız olgu, 1 yıldır karın ağrısı yakınması sonrası batın sol üst kadranda kitle nedeni ile yatırıldı. Batında, sol üst kadranda sert, yaklaşık 6 cm. çapında kitle saptandı. Lenfadenopatisi ve hepatosplenomegalisi olmayan olgunun diğer sistem muayeneleri olağan idi. Görüntüleme yöntemleri ve kitlenin açık-eksizyonel biyopsi incelemeleri sonucunda kitleye pankreasın solid-psödopapiller tümörü tanısı konuldu. Total kitle eksizyonu yapıldı. Tümörde alfa 1 anti-kimotripsin ve nöron spesifik enolaz pozitif, vimentin, sitokeratin, alfa-feto protein, karsinoembriyonik antijen ve östrojen-progesteron reseptörleri negatif bulundu. Olgu, tümörde nükleer atipi ve venöz emboli saptanması nedeniyle -malign davranış yönünden risk grubunda olduğu için-karaciğer ve bölgesel lenf nodu metastazı açısından yakın izleme alındı. Hasta operasyon sonrası üçüncü yılındadır ve rekürrens ya da metastaz gözlenmemiştir. Cerrahi rezeksiyon sonrası prognozun oldukça iyi olduğu düşük dereceli bu tümörlerde doğru tanı önemlidir.

Abstract

Pseudopapillary tumor of the pancreas is a rare, unique clinicopathologic entity. Local infiltration, distant metastasis and recurrence is quite rare. it is generally recognized as a growing abdominal mass with related symptoms in a adolescent or young female. A fourteen-year-old female patient complaining of abdominal pain for one year was hospitalized for the finding of a mass on the left upper quadrant. A hard mass, approximately 6 cm in diameter was found. The patient had a normal physical examination, including the absence of lymphadenopathy and hepatosplenomegaly. The mass was diagnosed to be a solid Pseudopapillary tumor of the pancreas by means of imaging techniques and open exicional biopsy. The mass was totally excised. The tumor was positive for alpha 1 anti-chymotrypsin and neuron specific enolase; negative for vimentin, cytokeratin, alpha-feto protein, carcinoembryonic antigen and estrogen-progesterone receptors. The patient was concluded to be in high risk group in terms of malignant behavior because of the detection of nuclear atypia and venous emboli and therefore kept on close follow-up for liver and lymph node metastasis. The patient is now living her third postoperative year and neither recurrence, nor metastasis is observed. Correct diagnosis of this rare tumor is crucial, because the prognosis is quite favorable after surgical resection.
