Atriyoventriküler septal defektli olgularımızın kısa ve orta süreli takipleri

Özet

Bu çalışmanın amacı nomenculatüre tanımlamasında farklılıklar olan atriyoventriküler septal defekt (AVSD)'in demografik, ekokardiyografik, hemodinamik ve cerrahi özellikleri ile yeniden irdelenmesidir. AVSD tanısı ile 1998-2003 yılları arasında izlenen 49 hasta retrospektif olarak değerlendirildi. Yaş ortalaması 33.7±43.8 ay olan olguların çoğunluğunu erkek çocuklar (%53.1) oluşturuyordu ve %30.6'sında Down sendromu mevcuttu. Rastelli sınıflamasına göre 31'i tip A, 1 olgu tip C ve 1 olgu da unbalanced tipte idi. Olguların 29'unda izole AVSD mevcut iken diğerlerine çeşitli konjenital kardiyak anomaliler eşlik ediyordu. Onaltı olguda mitral yetmezlik, 2 olguda sol ventrikül çıkış yolu obstrüksiyonu, 16 olguda pulmoner hipertansiyon (PHT) saptandı ve bunların 8'inde PHT sistemik düzeydeydi. Olguların 22'si (%44.9) opere edildi. Vakaların 8'ine pulmoner arter bantlama, 8'ine tam düzeltme ve 6'sına Blalock-Taussig (BT) şant operasyonu yapıldı. opere olan olguların operasyon sonrası takiplerinde ortalama 7.4 ayda kilolarında operasyon öncesi kilolarına göre %26.9'luk bir artış saptandı. Takipte öpere edilen vakalarda ölüm gözlenmezken öpere edilmeyen bir vaka kardiyak yetmezlikten eks olmuştur. Tam düzeltme yapılan üç olguda mitral yetmezlik, bir olguda hemodinamik yönden önemli rezidü VSD saptanmış; iki olgunun da şantı takipte tıkanmıştır. Kalp yetersizliği nedeniyle ölümlere yol açabilen ve sağ kalanlarda ise obstrüktif akciğer hastalığı yapabilen bu hastalıkta erken tanı, tedavi ve uzun süreli izlem ile iyi sonuçlar elde edilebileceği kanısındayız.

Abstract

The aim of this study to investigate again the surgical hemodinamic, demographic and echocardiographic properties of atrio-ventricular septal defect (AVSD) that has differences in description of nomenculatüre. Fouriynine patients that have diagnosis of A VSD between 1998-2003 years are evaluated retrospectively. Mean age of cases most of whom are boys (53.1 %) is 33.7+43.8 months and in 30.6% there was Down syndrome. According to Rastelli classification 31 of them were type A, one case was type C and one case was unbalanced type. Twentynine of cases there was isolated AVSD and extracardiac congenital anomalies were accompanieing to the others. Mitral insufficiency in 16 cases, obstruction of left ventricular output in 2 cases, pulmonary hypertension was systemic in 8 of these cases; 22 of cases (44.9%) were operated. To 8 of the cases pulmonary artery banding, to 8 of the cases total correction, and to 6 of the cases Blalock-Taussig shunting operations were done. In the follow up operated cases after operation, there was 26.9% increase in the weight in average of 7.4 months comparing to the preoperation weight. While in follow up no death is observed in operated cases, there was one exitus because of cardiac insufficiency in one of the unoperated case. Three of totally corrected cases had mitral insufficiency and one cases had hemodinamically important residual VSD; in follow up shunt of 2 cases were plugged. With early diagnosis, treatment and long lasting follow up in this disease that can cause death because of cardiac insufficiency and can cause obstructive lung disease in survivors, we have opinion that results can be achieved.
