Özet

Gram-negatif basillere bağlı septisemide ağız ve cilt bulguları tanımlanmıştır. Bu lezyonlar en sık Pseudomonas aeruginosa bakteremisinde görülmektedir. Yenidoğan servisimizde izlenmiş dört olguda nekrotik ağız ve/veya cilt lezyonları (noma neonatorum veya ecthyma benzen lezyonlar} birlikteliği olan Klebsiella sepsisi saptanmıştır. Olgularımızda olduğu gibi nekrotik cilt lezyonları daha az sıklıkla da olsa, diğer gram-negatif bakteremilerde de görülebilmektedir. Klinisyenin öncelikle sistemik gram-negatif basil infeksiyonu olasılığı üzerinde durmasının, sonra da özgül etiyolojik tanıya gitmesinin gerekliliğini vurgulamak üzere olgular sunulmaktadır.

Abstract

Oral and/or skin involvement during gram-negative bacillary septicemia have been described. These lesions have been most commonly associated with Pseudomonas aeruginosa bacteremia. The association of Klebsiella sepsis with necmtic oral and/or skin lesions (noma neonatorum or ecthyma-like skin lesions) in four patients followed-up in our neonatology unit was identified. Occasionally, necrotic skin lesions may be observed during other gram-negative bacteremias, as observed in our patients. These cases are presented in order to emphasize that the clinician should initially consider the possibility of a systemic gram-negative bacillary infection, and subsequently make a specific etiological diagnosis.
