Diabetik ketoasidoz tablosunda başvuran çocukların klinik ve laboratuvar değerlendirilmesi

Özet
Bu çalışmada, Diabetes Mellitus'un önemli bir komplikasyonu olan diyabetik ketoasidoz nedenlerinin, ketoasidoz sırasında gelişen biyokimyasal değişikliklerin, tedaviye cevap ve tedavi sırasında gelişebilecek komplikasyonların irdelenmesi amaçlanmıştır. Dr. Behçet Uz Çocuk Hastanesi'nde 1999-2001 tarihleri arasında acil servise başvuran ve diyabetik ketoasidoz tanısı ile tedavi gören 56 hastanın klinik ve laboratuvar bulguları incelenmiştir.Hastaların yaş ortalaması 9.7 ± 3.3 yıl olup, %54'ü kız (30) ve %46'sı erkek (26) idi. 39 hastada diyabet tanısı yeni konmuşken, 17 hastanın ortalama 45.7 ± 26.3 aydır diyabet tanısı ile takip edil-diği saptanmıştır. Diyabetik ketoasidoza yol açan nedenler arasında enfeksiyonlar ilk sırada gelmiştir (%67). 56 hastanın 10'unda (%18) hiponatremi, 12'sinde (%21) hipopotasemi, 51'inde (%91) hiperosmolarite saptanmıştır. Hastaların başvuru sırasında sodyum, potasyum, klorür, üre, bikarbonat ve düzeltilmiş sodyum ortalama değerleri ile 24. saat değerleri karşılaştırıldığında istatistiksel olarak anlamlı bir fark olmadığı görülmüştür (p > 0.05). Ancak buna karşın kan şekeri ve osmolaritenin ortalama değerlerinde 24 saat sonra istatistiksel olarak anlamlı düşüşler saptanmıştır (p < 0.05). Hastaların IV insülin tedavisinden SC insülin tedavisine geçiş süresinin ortalaması 15.3 ± 8.3 saat olarak saptanmıştır. Hastaların hiçbirisinde beyin ödemi ve hipoglisemiye işaret edecek semptom ve bulgulara rastlanmamıştır. Diyabetik ketoasidozun, olguların geç başvurmalarına rağmen, uygun tedavi ile kısa sürede ve komplikasyonsuz olarak tedavi edilebildiği gösterilmiştir.
Abstract
In this study, we purposed to examine the diabetic ketoacidosis as a complication of diabetes mellitus, the biochemical changes and the reasons of this complication, the answer to the treatment and the complications that occur during the treatment. The clinical and laboratory findings of 56 children with diabetic ketoacidosis who had been attended to the Emergency Service of Dr. Behçet Uz Children' s Hospital between 1999 - 2001 were evaluated. The mean age of the patients was 9.73+ 3.3 years. There were 30 (54%) female and 26 (46%) male. The diagnosis was new in 39 patients but the rest of them were diabetic meanly 45.7 + 26.3 months. Among the reasons that caused diabetic ketoacidosis, infections had come the first (67%). Out of 56 patients, 10 of them (18%) had hyponatremia, 12 of them (21 %) hypokalemia and 51 of them (91%) hyperosmolarity. As we compared the mean values of sodium, potassium, chloride, urea, bicarbonate and corrected sodium during the attendance time (0. time) and 24. hour, we did not find statistically significant difference (p > 0.05). Despite of this result, after 24 hours, blood glucose and osmolarity mean values decreased significantly (p < 0.05). The mean transition time from IV. therapy to the subcutaneus therapy was 15.3 ± 8.3 hours. None of the patients had the symptomes and signs that point the brain edema and hypoglycemia. We showed that, with an appropriate therapy, the diabetic ketoacidosis could be treated in a short of time and without any complication, despite of late attendance to the hospital.
