Özet

Gözlemler ve araştırmalar sağlık çalışanlarına karşı sıklıkla şiddet uygulandığını göstermektedir. Ülkemizde yapılan çalışmalarda acil serviste gözlenen şiddet olgusunda, sözel şiddetin fiziksel şiddetten çok daha yaygın olduğu saptanmıştır. Ocak-Nisan 2005 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Çocuk Acil Servisinde çalışan doktor, hemşire ve personel üzerinde yapılan bu çalışmada şiddet olgusunun yaşanıp yaşanmadığı, ne sıklıkla yaşandığı ve şiddetin özelliklerinin araştırılarak şiddetin önlenmesi için, şiddetin nedenleri hakkında ipuçları elde edilmesi amaçlanmış ve önerilerde bulunulması planlanmıştır. Çalışma süresi boyunca Pediatrik Acil Servisinde çalışan doktorlara %53.4, hemşirelere %37.9 ve personellere %8.6 olmak üzere sağlık çalışanlarına karşı toplam 58 şiddet olgusu yaşanmıştır. Elli sekiz şiddet olgusunun %94.8'ini sözel %5.2'isini ise fiziksel şiddet oluşturmuştur, şiddetin en çok 16:00-24:00 saatleri arasında, en sık anneler tarafından uygulandığı bulunmuştur. Şiddet olgularının %37.9'unun hastalara girişim esnasında, %25.9'un da muayene sırası beklerken yaşandığı saptanmıştır. Şiddetin uygulandığı sırada, acil servisin yoğunluğu değerlendirildiğinde, yoğunluğun fazla olduğu dönemlerde şiddetin daha sık olduğu gözlenmiştir. Olguların başvuru yakınmalarına bakıldığında, solunum sıkıntısı, havale geçirme ve ateş ile başvuran olguların ailelerinin daha sıklıkla şiddet uyguladıkları saptanmıştır. Ege Pediatrik Acil Servisinde gözlenen şiddetin önlenebilmesinde, hasta çocuğun durumunun aileye uygun bir şekilde bildirilmesi, yoğunluğun önüne geçecek önlemlerin alın¬ması, başvuru kayıt işlemlerinin basitleştirilmesi ve çalışma ortamında, ergonominin arttırılmasının yararlı olacağı düşünülmüştür.

Abstract

Violence in Emergency Departments is a commonly observed and reported phenomenon. Reports from our country demonstrate that verbal violence is much more common then physical violence. Between January-2005 and April- 2005, we applied a questionnaire to pediatricians, nurses and the other staff working in Emergency Department of Ege University. The aim of this study was to determine the characteristics and prevalence of the violence in Emergency Department of our Hospital and detect the clues to solve this problem. During the study period 53.4% of the doctors, 37.9% of the nurses and 8.6 of the other staff working in Emergency Department reported that they were exposed to violence at least one time. Overall 58 events of violence happened and 94.8 % of them were verbal and only 5.2% of them were physical violence. The events of the violence were mostly committed by the mothers of the children and most commonly observed between the hours 4pm and 12 pm of the day. The events were occurred mostly while the children were being examined (37.9%) or they were waiting for examination (25.9%). The parents whose children had respiratory distress, seizure or fever more commonly committed violence against the health care team in Emergency Department. As a conclusion, violence particularly verbal violence is a common problem in Emergency Department's. Some prevention methods seem to be helpful to decrease violence rate. Simplification of administrative procedures, carefully triage of the critical patients by the experienced nurses and giving proper information to the parents may be the initial steps.
