Özet

Tip 1 diyabet otoreaktif T hücreleri aracılığı ile pankreas beta hücrelerinin otoimmün hasarı sonucu oluşan kronik bir hastalıktır. Geçtiğimiz 10 yılda araştırmacılar tip 1 diyabetteki otoimmün olayın önlenmesi ve tedavi edilmesine odaklanmışlardır. Hayvan modelleri üzerinde yapılan çalışmalar insulin, GAD65 ve bunlardan elde edilen peptitler gibi spesifik adacık hücre proteinleri ile yapılan aşılamanın diyabet insidansinda azalma ve hastalık bulguları başlamış olsa bile hastalığın ilerlemesini önleyebileceğini göstermiştir. Her ne kadar bu metotlar hastalığın insidansı ve şiddetinde azalma yapsa da, çalışma yapılan hayvanların çoğunda ciddi ve ani ölümle sonuçlanabilen anaflaktik reaksiyonlar bildirilmiştir. Son olarak Tip 1 diyabetin önlenmesi ve tedavi edilmesinde spesifik CD4+ ve CD25+ regulator T hücreleri ile aşılama gündeme gelmiştir, ilk sonuçlar umut vericidir ancak T hücre aşılamasının Tip 1 diyabetli hastalarda kullanılması için daha ileri çalışmalara gereksinim vardır.

Abstract

Type 1 diabetes is a chronic disease which results from autoimmune destruction of pancreatic beta cells mediated by autoreactive specific T cells. During the past decade, investigators have focused on developing methods for preventing and treating autoimmune process in type 1 diabetes. Studies in animal models showed that vaccination with specific islet cell proteins such as insulin, GAD65 and their peptides derived from them can decrease the incidence of diabetes and prevent progression even if administered after the initiation of symptoms by causing T cell tolerance. Although these methods have resulted in significant decrease in incidence and severity of disease, serious side effects such as very high degree of anaphylactic reactions, leading to acute death have been also reported in the majority of treated animals. Recently, therapeutic vaccination with specific CD4+ and CD25+ regulatory T cells has become a current issue for preventing and treating type 1 diabetes. Initial results of these studies seem to be hopeful but for using of T cell vaccination in patients with type 1 diabetes, we need further investigations.
