Özet

Bazı kanserli hastalarda, primer tümörlerin sonraki takipleri esnasında sekonder tümörler ile karşılaşılmaktadır. Sekonder tümörlerin gelişimi morbidite ve mortaliteyi arttırarak prognozu kötüleştirmesi nedeniyle kanserli hastalar için önemli bir sorun teşkil eder. Çocukluk çağı kanserlerinde tanıdan sonraki yaşamın 10-20 yılı sekonder kanser gelişimi için risklidir. Kliniğimizde Hodgkin hastalığı (HH) ve akut lenfoblastik lösemi (ALL) tanısı ile izlenen iki olgu, .sekonder non-Hodgkin lenfoma (over-kemik) gelişimi yönünden sunulmuştur. Primeri HH olan olguya kemoterapi (ABVD) ve boyuna tutulan alan radyoterapisi uygulanmış, alkilleyici ajan kullanılmamıştır. Tedavi kesiminden 5.5 yıl sonra kraniumda B-hücreli non-Hodgkin lenfoma gelişmiştir. Erken pre-B hücreli akut lenfoblastik lösemili ikinci olguda BFM 90 standart risk protokolüne göre kemoterapi (siklofosfamid ve prednisolon içeren) verilmiş, kranial radyoterapi uygulanmamıştır. Bu olguda tedavi kesiminden 1 yıl sonra överde lenfoma gelişmiş ve T hücre belirleyicileri pozitif bulunmuştur. Kanserli hastaların uzun süreli izlemleri hem komplikasyonların hem de sekonder malignansilerin gelişmesi yönünden önemlidir.

Abstract

Some cancer patients develop secondary tumors after treatment of the primary tumors, or during their later courses. The development of secondary tumors worsens the prognosis by increasing morbidity and mortality, and thus constitute an important problem. Ten to 20 years of life after diagnosis of childhood cancers poses risk for the development of secondary tumors. Two patients with the diagnosis of Hodgkin's Disease (HD), and acute lymphoblastic leukemia in our clinics are presented for secondary development of non-Hodgkin's Lymphoma (NHL) The patient with HD had been previously treated with chemotherapy (ABVD) and radiotherapy of the involved neck region. This patient developed B-cell NHL in the cranium 5,5 years after the completion of the therapy. The second patient with early pre-B-cell ALL had been treated with chemotherapy (including cyclophosphamide and prednisoione) according to the BFM 90 standart risk protocol. This patient developed lymphoma in the ovary 1 year after completion of therapy and had positive T-cell markers. Long-term follow-up of cancer patients is crucial in terms of development of complications and secondary malignancies.
