Özet

Yabancı cisim aspirasyonları çocukluk çağı kazaları içinde en sık görülenlerdendir ve önemli bir mortalite ve morbidite nedenini oluşturmaktadırlar. Bu çalışmada İzmir Dr. Behçet Uz Çocuk Hastanesi Acil Servisi’ne bir yıllık dönemde yatarak yabancı cisim aspirasyonu tanısı alan 32’si (% 66.7) erkek, 16’sı (% 33.3) kız 48 çocuk değerlendirilmiştir. Çocukların 40’ı (% 83.3) 0-3 yaş arasındaydı. Hastaların başvurma şikayetlerinin % 62’sini öksürük oluştururken, %35’ünü morarma ve %33’ünü solunum zorluğu oluşturmaktaydı. Fizik muayene bulguları içinde ise akciğerler arası havalanma farkı ilk sıradaydı (% 62.5). Sekiz hastada (% 16.6) radyolojik görünüm normaldi. En sık görülen patolojik radyolojik bulgu obstrüktif amfizemdi (% 66.6). Kırk sekiz hastanın 45’ine (% 93.8) rijit bronkoskopi uygulandı. Kırk sekiz olgunun 44’ünden (% 91.7) yabancı cisim çıkarıldı ve bunların 22’si (% 50) sol ana bronşta idi. En sık saptanan yabancı cisimler ay çekirdeği (% 27.3), fıstık (%11.4) fasulye (%11.4) ve leblebi (% 9) idi. Yabancı cisim aspirasyonu ile sonuçlanan kazaların önlenmesi için aileler, özellikle 3 yaş altındaki çocuklara çerez, kuruyemiş ve diğer küçük parçalı yiyecek ve şekerlemeleri vermemeleri konusunda uyarılmalıdır.

Abstract

Foreign body aspiration is one of the most frequent accidents in pediatric population and an important cause of childhood morbidity and mortality. We retrospectively evaluated 48 pediatric patients (32 boys, 16 girls) admitted to the Department of Pediatric Emergency, İzmir Dr. Behçet Uz Children’s Hospital over the past year. Fourty children (83.3 %) were between 0 and 3 years of age. Sudden onset of cough (62.5 %), cyanosis (35.4 %) and dyspnea (33.3 %) were the predominant symptoms and signs. The most common physical finding was asymmetric auscultation (67.5 %). Obstructive emphysema (66.7 %) were the most frequent radiological finding with four patients (16.7 %) having normal chest radiograph. Fourty five of 48 patients underwent rigid bronchoscopy. Foreign bodies were found in 44 (91.7 %) of 48 children and 22 of them (50 %) were in the left main bronchus. Sunflower seeds (27.3 %), nuts (11.4 %), beans (11.4 %) and chickpea (9 %) were the most commonly aspirated foreign bodies. The best way to reduce the number of accidents and deaths associated with foreign body aspiration is to educate the parents about the hazards of exposing children below 3 years to snacks and other food in small pieces.
