Çocuklarda metoklopramid kullanımına bağlı distonik reaksiyonlar

Özet

Metoklopramid hem antiemetik olarak hem de gastroözofageal reflü tedavisinde yaygın olarak kullanılan bir dopamin reseptör antagonistidir. Pediatrik yaş grubundaki üst solunum yolu enfeksiyonları ve gastroenterite bağlı bulantı ve kusmaların tedavisinde de sıkça reçete edilmektedir. Metoklopramidin en önemli yan etkilerini önerilen tedavi dozlarında bile görülebilen ekstrapiramidal bulgular oluşturmaktadır. İzmir Dr. Behçet Uz Çocuk Hastalıkları ve Cerrahisi Eğitim ve Araştırma Hastanesi Acil Servisine başvuran yaşları 3 ay ile 14 yaş arası değişen ve metoklopramid kullanımına veya aşırı dozuna bağlı yan etkiler gelişen 18 olgu sunulmuştur. Bu olguların 17’sinde distonik reaksiyonlar ön planda iken bir olgu bulantı, kusma ve konvülzyon geçirme şikayetiyle başvurmuştur. Ekstrapiramidal bulguları ortadan kaldırmak için 10 olguya tek doz, 4 olguya iki doz biperiden (Akineton®) yapılmış, bir olguya konvülzif nöbet geçirdiği sanılarak intranazal midazolam uygulanmıştır. İki olgunun bulguları ise spontan gerilemiştir. Gelişen bulgular ailelerde büyük bir panik ve korku, çocuklarda huzursuzluk yaratmaktadır. Acil servislerde çalışan hekimler ise metoklopramide ait distonik reaksiyonların çocukluk çağında çok sık görülmediğini, görüldüğü zaman da başka hastalıklarla kolaylıkla karışabileceğini akıldan çıkarmamalıdırlar.

Abstract

Metoclopramide is a dopamine receptor antagonist that is widely used for the treatment of gastroesophageal reflux disease and as an antiemetic agent. It is also prescribed by pediatricians in nausea and vomiting caused by respiratory tract infections and enteritis in practice. The main side effects of metoclopramide are extrapyramidal reactions which are even seen at recommended doses. We report 18 cases presented to the emergency department with the adverse effects of metoclopramide, even though the patients had used the drug in both normal or toxic dosing ranges. The dystonic reactions were leading in seventeen of the patients when one of the patients was referred to with complaining nausea, vomiting and seizures. Biperiden was administered as a single dose to ten patients and double doses to four patients in order to eliminate the adverse effects of metoclopramide. One patient was administered intranasal midazolam because the dystonic reactions due to metoclopramide was confused with seizures. Two of the patients were recovered spontaneously. The symptoms arisen actually create a great panic and horror in families and anxiety in children. Physicians who work in the pediatric emergency department must be aware of dystonic reactions caused by metoclopramide and should never forget that these infrequent reactions can be easily confused with other diseases
