Özet

Amaç: Biz bu çalışmada acil servise başvuru anında alınan sürüntü örneğinde RT PCR testi negatif gelmesine rağmen tipik klinik belirtiler ve toraks bilgisayarlı tomografi (BT) bulguları nedeniyle koronavirus hastalığı (COVID-19) için izole edilip hastaneye yatırılan ve tekrarlayan sürüntü örnekleri pozitif gelen hastaları değerlendirmeyi amaçladık. Gereç ve Yöntemler: Başvuru sırasında toraks BT‘ de koronavirüs hastalığı 2019 (COVID-19) için tipik bulguları olan ve ilk başvuruda RT-PCR testi negatif olan ancak ikinci RT-PCR testi pozitif olan hastaları retrospektif olarak inceledik. Bulgular: Toplam 12 hasta kayıt altına alındı. Toraks BT' de en sık görülen paternler %100 bilateral infiltrasyon, %83,3 buzlu cam opasitesi, %83,3 akciğerin üst, orta ve alt bölge tutulumu ve % 66,7 hem santral hem de periferal yerleşimli infiltrasyondur. Total ciddiyet skorunun (total severity skor) mortalite tahmininde ROC analizi ile belirlenen eşik değeri olan 10 değeri, mortalitenin tahmini için% 66.7'lik bir duyarlılığa ve% 100'lük bir özgüllüğe sahipti. Eğri altında kalan (AUC) değeri 0,944'tü (% 95 CI: 0,808-1,000, SE: 0,070, p = 0,027). Sonuç: Bilgisayarlı tomografi, COVID-19 tanısında önemli bir tanı aracı olarak varlığını kanıtlamıştır. Hastaların RT-PCR sonucu negatif olsa bile klinik şüphe ve tipik göğüs BT bulguları varlığında hastalar COVID-19 açısından izole edilmeli ve tedaviye erkenden başlanmalıdır.

Abstract

Objectives: We aimed to evaluate the patients who were isolated andhospitalized for coronavirus disease (COVID-19) due to the typical clinicalsymptoms and chest computed tomography (CT) findings, although the realtime reverse transcriptase-polymerase chain reaction (RT-PCR) test was negativein the swab sample taken on admission emergency department, and who hadpositive results in repetitive swab samples.Methods: We retrospectively reviewed the patients who had typical findings forcoronavirus disease 2019 (COVID-19) on chest CT on admission, and whose RTPCR test at the first admission were negative, but the second RT-PCR test waspositive.Results: A total of 12 patients were enrolled. The most common patterns onchest CT were 100% bilateral infiltration, 83.3% ground-glass opacity, 83.3%upper, middle and lower zone involvement, and 66.7% both central andperipheral located infiltrations. In receiver operating characteristic (ROC) curveanalysis an optimal cut off value of a total severity CT score of 10 had asensitivity of 66.7% and a specificity of 100% for the prediction of mortality. Thearea under the curve (AUC) was 0.944 (95% CI: 0.808-1.000, SE:0.070, p=0.027).Conclusions: Computed tomography has proven its existence as an importantdiagnostic tool in the diagnosis of COVID-19. Even if the RT-PCR result of thepatients is negative, in the presence of typical chest CT findings with clinicalsuspicion, patients should be isolated in terms of COVID-19 and treatmentshould be initiated early.
