Özet

Amaç: Anorektal malformasyonlu çocuklarda postoperatif anal darlığı önlemek için anal dilatasyon programı uygulanır. Bununla birlikte, inatçı bir darlık geliştiğinde, cerrahi genellikle iyi bir seçenektir. Tanımlanan birkaç teknik olmakla birlikte, uygulama konusunda evrensel bir fikir birliği yoktur. Burada, çocuklardaki cilt seviyesi anal striktürleri ve doğuştan anal darlıklarda uygulanabilecek basit bir prosedür olan Y-V anoplastiyi tanımladık. Yöntem: 2013-2019 yılları arasında Y-V anoplasti uygulanan hastaları retrospektif olarak inceledik. Hasta litotomi pozisyonunda iken, Y'nin dikey kolunu oluşturan ilk kesi saat 6 pozisyonunda darlık alanını içerecek şekilde yapıldı. Bu kesi perianal deride iki yönde uzatılarak tam kalınlıkta V şeklinde bir flep oluşturuldu. V ilerletme flebinin apeksi, tek tek emilebilir sütürler ile yeni bir mukokütanöz bağlantı oluşturacak şekilde Y insizyonunun dikey koluna dikildi. Bulgular: Çalışma süresi boyunca altı hastaya Y-V anoplasti uygulandı. Y-V anoplasti sırasında ortalama yaş 5,08±2,69 (medyan:6) yıl idi. Dört hasta rektovestibular fistül ve bir hasta rektoprostatik fistül nedeni ile daha önce opere edilmişti. Kabızlık şikayeti belirgin olarak düzelen anterior ektopik anüsü olan bir hastaya ilk ameliyat olarak Y-V anoplasti uyguladık. Preoperatif olarak dışkı kaçırması olan beş hastadan dört tanesi tamamen iyileşirken, bir tanesinde az miktarda düzelme görüldü. Sonuç: Y-V anoplasti, cilt seviyesi anal darlığı ve doğuştan anal stenozu olan çocuklarda uygulanabilecek basit bir cerrahi işlemdir. Fekal kontinansı ve kabızlığı iyileştirmede de katkıda bulunabilir.

Abstract

Aim: A regimen of anal dilatations is performed to prevent postoperative anal stenosis in children with anorectal malformations. Nevertheless, when an intractable stricture develops, surgery is usually a good option. Several techniques have been described, but there is no universal consensus. We here described a simple procedure for skin level anal strictures and congenital anal stenosis in children. Methods: We retrospectively reviewed patients who underwent Y-V anoplasty from 2013 to 2019. With the patient in the lithotomy position, the first incision that forms the vertical limb of the Y is made at the 6 o'clock position to include the area of the stenosis. This incision is extended on the perianal skin in two directions for creating a full-thickness V-shaped flap. V advancement flap is sutured into the vertical limb of the Y incision to create a new mucocutaneous junction with interrupted long-term absorbable sutures.Results: Six patients underwent Y-V anoplasty over the study period. The mean age at the time of Y-V anoplasty was 5.08±2.69 (median:6) years. Four patients had previously been operated for rectovestibular fistula and one patient for rectoprostatic fistula. We performed Y-V anoplasty as a primary operation in a patient with anterior ectopic anus whose constipation signifintly improved. While 4 of 5 patients who had fecal incontinence preoperatively became fully continent, one had slightly improved. Conclusion: The Y-V anoplasty is a simple surgical procedure that can be performed in children with skin-level anal strictures and congenital anal stenosis. It may also contribute to improve fecal continence and constipation.
